3. OLD BUSINESS (from Minutes of March 15, 1978 PMSB Meeting)
II. Accident Analysis Weekly Bulletin--Glass
The weekly bulletin is being sent to members of the Plant Manager's expanded staff each Friday.
III. B. Guards For Smooth Rotating Shafts -- Ryan
The committee has reviewed the information received from a number of sources and they plan to report at the next meeting of the Board.
III. D. Railroad Lighting -- Danner
Last month's meeting minutes incorrectly listed the Distribution Department being responsible for issuing the PAFE for additional lighting. Bill Riddick, who is responsible for the railroad tracks, will prepare the request for Engineering Services for the Appropriation Request. The Engineering estimate for the improved rail road lighting is being prepared in Engineering Services.

Enlarged Representation on Housekeeping Inspections -- Triplett
Chairman ofc SHAC Committee, Bill Triplett, proposed the addition of a foreman or first line supervisor to the inspection teams making the quarterly guest housekeeping inspections. The proposal was approved by the Board and will be incorporated into the procedure used for the next inspection.
IV. D. Hose Task Force Report Status -- Triplett
The SHAC committee proposed approval of two types of fittings to be used on hose connections as an interim step prior to the issuing of a more detailed definitive procedure bulletin on all aspects of hoses used throughout the plant. The target date for the overall plant hose procedure bulletin was set for July 15, 1978.
IV. E. 1) Basic Safety Rule Training Status
The various members of the Board reviewed the status of their supervisory safety rule training. It is expected to be completed plantwide by the end of April.
2) Basic Safety Training for Foremen -- Glass
The pilot course for foreman safety training has been completed. Comments have been received and revisions will be made before the next offering, tentatively scheduled for June 1978.

IV. E. 3) Job Safety Audits/Job Hazard Analyses (Status) -- Glass
The basic training program for the unit safety promotion teams on these two safety analytical concepts has been delayed due to STEP-SAF training. It is now planned for May 1978.
IV. E. 4) Critical Reviews
A plantwide system for scheduling critical reviews through the plant work order system is being prepared by Paul Bailey and Arnold Glass. A report to the Board is tentatively scheduled for the May 1978 meeting of the Plant Manager's Safety Board.
V. A. Engineering Design Specification for Attachments to 650# Steam Systems -- Danner
Threaded connections should not be used on 650# steam systems. Anyone making changes in revisions on steam system should check the Corporate Engineering Standard as recorded in Plant Engineering Services. SHAC was requested to put out a plant notice to remind everyone in the plant of this requirement: NOT TO HAVE THREADED CONNECTIONS ON 650# STEAM SYSTEM.
V. Property Loss Incident Notification and Reporting -- Metten
The new Property Loss P-Bulletin reporting procedure is now in effect. Anyone having problems in interpreting it should contact Arnold Glass or Doug Metten. If training on its use is required, training sessions will be scheduled.
VI.
Superhazardous Fluid P-Bulletin -- Trafton
The final copy of the P-Bulletin on Superhazardous Fluids is being typed for distribution for comments. Target date for final approval is next meeting of PMSB.
4. REPORT OF SAFETY ADVISORY COMMITTEE (SAC) -- G. T. Ryan
The committee studying problems related to the controlled use of ground fault indicators (GFI's) are nearing a conclusion of their study. They have had an expert in this field in the plant consult ing with them and they feel that they will have a proposal ready for review by the next PMSB meeting.
5. REPORT OF SAFETY HOUSEKEEPING AND ACTION COMMITTEE (SHAC) -- W. H. Triplett
a. Emergency Procedure Plan
A test of the Emergency Procedure Plan is being developed and will be on the agenda of the next meeting of the PMSB. Tenta tively, plans are to have a plantwide test in late May.

b. Annual Housekeeping Program
Six committees are being formed to handle the details of the annual housekeeping inspection. SHAC is developing proposed staffing of the committees and members of the Board will be. be contacted for appointees. The work of these committees will start in May 1978.
c. Hourly Safety Committee Meeting
The chairman of SHAC and an alternate and Tony Ozarchuk met with the Hourly Safety Committee, reviewed their concerns, are investigating the items presented and are expecting to resolve them in an orderly way.
d. SHAC Activity Status Reports
Contrary to the statement of last month's PMSB meeting minutes, the SHAC activity status report will be issued quarterly to PMSB members, instead of monthly.
