Comments on Statistics--R. T. Hammann Although there were no Recordable Injuries In January, we had our first Recordable Injury on February 1. This occurred to an electri cian who sustained a laceration requiring three stitches. Areas continuing to have high injury rates are: Pumper Gaugers, Painters (although one injury is significant in this small group). sc 13977 STG 2430783 LAM015766 PMSB Meeting Minutes--February 16, 1983 Page 2 Dept. 44/45/46, Dept. 50, and Analysts. Of special concern is the Analysts group as they have shown an increasing high injury rate in the last five months. The Electricians are at a higher rate than in the past several years, and had the Recordable Injury in February. Areas showing excellent injury performance are: HCN Complex, Dept. 48/56, Dept. 53, and Dept. 513. The Stores Dept, and Dept. 33 deserve special recognition as neither of these departments have had an Injury in the past year. C. Contractor Safety--G. A. Danner One Recordable Injury was reported in January. This occurred to an Opel Pipefitter who fractured the third finger of his right hand when his hand was caught between two pieces of four inch pipe. D. CED Project Safety--B. C. Lancaster No Injuries occurred during the small number of hours worked this month. This will probably be the last report from CED as work is winding down toward a cessation of all activities. II. REPORT OF THE SAFETY ADVISORY COMMITTEE (SAC)--D. E. Kaldenberg A. S&PP Recommendation 82-2 on Bnergency Shutdown/Standby Procedures Procedures will be written for each unit (including laboratories and Material Handling) for placing a unit in safety condition so that personnel can evacuate immediately. It will be assumed in these procedures that personnel can return to the area within one hour. These procedures will be completed and reported in writing to G. A. Danner prior to the March 16, PMSB meeting. B. Survey of Low Pressure Steam Header (Boots) MTS proposed a spot inspection of twelve steam boots, leads, and con nections on the ex-battery 75 psi steam header where a failure recently occurred. This represents approximately 10Z of the total boots and connections on the header. Dependent on the findings of this inspec tion, more work may be done until all serious corrosion areas are repaired. This approach was approved by PMSB. IH. REPORT OF THE PLANT SAFETY AND HOOSEKEEPING ACTION COMMITTEE (SHAC)-- A. H. Conrad A. Hourly Safety Committee 13978 Transition meeting was held between the incoming and outgoing members with eleven of the twelve in attendance. Manuel Perez was elected as the new chairman of this committee. The new members were briefed on the purpose of the Hourly Safety Committee by Mark Riddle. Gene Tromblee reviewed the 1982 plant safety highlights. STG 2430784 LAM015767 PMSB Meeting Minutes--February 16, 1983 Page 3 Concerns: 1. Inventory in phenol hose at Dock 2. This was satisfactorily resolved. 2. Cooling Tower Fan Hazard - The problem is development of a safe procedure/method for stopping rotation of the fan blade prior to entry into the tower. Charles McClanahan will followup. 3. Safety Log Books - Some concern over the fact that safety items are not always completed in a reasonable amount of time. A memo requesting better followup was written to supervision by Harry Conrad. 4. Removing Bolts From The 51D4 Manway Covers - This incident brought up the practice of removing manways from equipment that has not been completely blinded. SHAC will make a canvas to determine how wide spread this practice is in the plant. This Information will then be given to SAC for review to determine an overall plant policy. B. Eyebath/Safety Shower A drawing of the new plant standard has been Issued for final approval. C. Quarterly Inspections Bill Howe reviewed the plans for implementing the Competitive Quar terly Safety & Housekeeping Inspection in March. These inspections will be carried out the final two weeks of the month. Publicity will begin before the end of February. D. Procedure Bulletins Three bulletins that have undergone minor revisions in policy have been approved and will be issued in the near future. These bulletins are: P-0116, Area Safety Inspections; P-0121, Use and Control of Hoses; and P-0123, Bypassing and Testing Protective Devices. Only the page containing the policy revision will be issued. IV. GENERAL PERMIT PROCEDURE--R. T. Hammann Final revisions have been made and approval signatures are being obtained. Training sessions and implementation of the procedure will be delayed until permits can be printed. V. FIRE AND ENTRY PERMIT PROCEDURE--R. T. Hammann Permits will be received soon from the printers. REVIEW OF INCIDENTS Monsanto Incidents Date Incident No. Description 01/21/83 83-01 (not issued--has been signed and will be Issued in the near future) Spill--Dept. 13; flange leak on top of alkylator; spill of HC1 and Bz vapors; NO EXPOSURES. (Furley) 02/01/83 83-02 (issued) Recordable Injury--Dept. 53; Electrician caught second finger of left hand in swab of drill requiring three stitches to close. (McVay) NOTE: This is the second Recordable Injury of the same type (broken bone due to failure to unplug equipment while working on it) that has occurred in the plant in the last 4-5 months. Steps should be taken to convey this to all personnel in the plant. STATUS OF S-T-O-P PROGRAM--M. A. Riddle The second group of Chiefs/Leadmen has gone through the first unit of S-T-O-P training. Training has progressed well with much Interest in the program. VIII. PLANT MANAGER'S COMMENTS--S. G. Pappas for G. L. Tromblee Our injury record so far this year is very good. However, we need to take steps to ensure that we do not become complacent.
