(b) On May 18, 1978, Mr. Richard Miller, Industrial Hygienist with OSHA, was notified by phone that we had experienced a VCM release at our ftb Plant evacuation compressor which resulted in exposure to seven (7) of our employees. At 2:30 P.M., without prior indications that he would visit the plant, Mr. Miller arrived at the Personnel office and requested an interview with me. Steve Brouillard and myself met Mr. Miller who officially identified himself and informed us that he was here to investigate the VCM re lease and the scene of the incident. During the initial phase of the interview Mr. Miller asked to see the records of our 1977 work injury experience, OSHA Logs 100, 101, 102 were examined by Mr. Miller who indicated that he was interested in injuries relating to liver ailments and injuries relating to PVC production, etc. After the inspection of the OSHA Logs Mr. Miller asked questions con cerning our safety and medical programs and requested the name of the company president and the address on the Diamond Shamrock main office. Mr. W. Bricker's name was given as the company president. After the routine questions were completed, Mr. Miller requested an inspection of the compressor area. Steve Brouillard and myself then brought Mr. Miller down to Plastics where we introduced him to Rod Becker. Mr. Miller then explained to Rod the reason for the visit and displayed his credentials. Mr. Miller stated that he had inspected another PVC plant in this area on May 17, 1978, and that he was knowledgeable about PVC operations. Mr. Miller stated that he was instructed by his super visor to investigate this incident. Diamond Shamrock OCC 11114 R. P. Becker -3- May 19, 1978 Mr. Miller than stated that he was concerned about detecting the VCM and that he may need to conduct additional sampling at a later date using sample pumps and charcoal. Mr. Miller than said that he would like to leave for the day but would like to set up an interview for Monday, May 22, 1978, with the employees that were involved in the exposure. Since these employees are working the graveyard shift, Mr. Miller stated that he would arrive at 0600 A.M. on Monday to conduct the interview. Mr. Miller, just prior to leaving, stated that he would call me on May 19, 1978, and inform me if additional information was necessary.
