Before he got back to the poly rupture disc and relief valve vented. 4. Poly vented for approximately 6 minutes before pressure was reduced enough to resets RV,, Probable cause: Poly was over charged with either vinyl monomer or water. Corrective action: Operator failed to react properly to high pressure alarm situation. Poly wouldn't have vented if he had shut steam spurge off and cooled poly down and then began to recover poly soon as possible.
