Smith / Tokio Lease Inspection Date: 11/07/2019 PURPOSE OF THE INSPECTION EPA Region 6 inspector K.W. Sanborn visited the Roger G. Smith oil and gas facility located at latitude 33.535167 and longitude -93.156737 near Waterloo, Arkansas on 11/07/2019 for an unannounced inspection. There were no representatives of Roger G. Smith present for the inspection. The inspection was conducted by authority of the Clean Water Act and the Safe Drinking Water Act to determine compliance with the Environmental Protection Agency (EPA) regulations. FACILITY DESCRIPTION This facility is known as the Tokio tank battery and is located in the Troy oil field. Section II - OBSERVATIONS I observed surface damage east of the tank battery. I saw oil and brine in the secondary containment pit at the tank battery. Section III - AREAS OF CONCERN There was oil and brine in the secondary containment area and evidence of surface damage outside of the secondary containment area. Section IV - FOLLOW UP EPA water enforcement will refer this facility to EPA SPCC program and will conduct a follow up inspection in approximately 6 months.
