Reports .. i ____ The Monthly Safety and Housekeeping Audits and Injury Summary for October were reviewed: All areas reported to be in control of open container use of "Varsol". "No smoking" signs at the. truck unloading spot enroute to the Alathon emergency gate are in place only during tank truck unloading. Missing relief valve mating flange identification is wide spread as the tags tend to become hidden by paint and insulation or have broken loose. Improper storage of solvents, flammables and aerosols needs tighter plant control, There are indications that we need to crack down on plant seat belt requirements. Some method needs to be devised to keep seat belts from slipping behind the seats. ; Many items detected are of the type that result from a lack of attention to details that should have been corrected by the person doing the job. Housekeeping of outlying areas was excellent.' Training is needed in the detection of nonstandard safety glasses. There were no high potential injuries in November. Congratulations to Stores, HMD Maintenance Shop, "F" Unit Synthesis, Acids Power Water Treatment, and 524 Bldg, as Good Housekeeping Award winners. It was pointed out that 90% of the areas received an excellent rating vs. 62% year to date and 53% for 1977. 1 TI. Incident Reports Reports 78-65 through 78-74 were reviewed: 78-65 - There was a question whether a check list approach might help solve this problem. It was felt that the job was not too complicated and should have been valved properly. 78-66 - A thorough investigation^and not seen as having high potential consequences." One problem was getting on equipment with ;bad brakes. Another was the poor condition of the stop log. Equipment should have a tag or a means of communicating that there are mechanical problems. Proprietors of equipment should set up a basic functions check list. 78-69 - Illustrates a good piece of detective work in pinning down the silencer. This high potential incident resulted from poor design and poor quality workmanship. 78-70 was high potential because people are always looking into dumpsters. More attention to supports is needed plant wide in design, installation, and checkout'. 78-73 - It was emphasized that other plant;areas should determine if followup is required on vendor equipment. 78-74 - Hard wired interlocks vs. electronic or computer safety interlocks? The interlocks will be looked;at and a Victoria Plant Policy statement defined. III. Safety Activities Committee - W. L. Walton; Jr. On Plant N'o lost or restricted workday cases ! Safety prizes have been ordered and should be arriving at employees' homes soon. Off Plant There were two off-the-job injuries in November that resulted in lost time. This makes a total of 11 :for the year vs. our goal of no more than 10. The Off-Plant Bingo Contest has not accomplished its objective. The program will continue until the Safety Activities Committee acts on its program for 1979. ' IV. Revisions to S 6 F 48, 49 8 57 - J, L. Chambar.d Revision of these procedures is necessary in order to include: Relief pressure factors for pressures above 3000 psi Add section for inlet line pressure loss; Make reference to VP Std. 8A DUP 0801436 DU 007903 Victoria Plant Central Safety Committee Minutes of Meeting Held December 12, 1978 Page 4 Add section for design dynamic thrust calculation. The changes were approved, with Chambard to furnish forms for safety manual revision. V. Incident Report No. 78-62 - Jim Borden Jim Borden displayed a safety harness designed for use with the "Hose Line Respirator Breathing Apparatus" used in Type II atmospheres. This system, used with the harness, is NIOSH certified. The suggestion to use the harnbss instead of the belt and lock type coupling recommended in! 78,-62 was approved. Harnesses will be secured and attached to these systems in the plant. VI. Fire Statistics - C. E , Bunn A chart is attached showing the number of fires per vear since 1973. VII.Key Safety Data -- | High potential incidents made a dramatic increase in October. The high rate plagued us again in November, We are in a year end slump and in need of a big push to get:this trend turned around. There have been too many high potential incidents. And off plant safety is also suffering. ; VIII. Breathing Air Cylinders W. P. Ledet An audit conducted in October, 1978 questioned whether breathing air cylinders should be cycled for refilling on some frequency. Winston's research concludes that this shouldibe done on a two year frequency, based on lab sticker inspection dates.
