This is basically what you have suggested. (a) The persons missing exams will be sent to a local medical facility for the following tests: pulmonary function, EKG, audiometry, vision, tonometry, x-ray, blood pressure, height and weight. These results will be sent to Dr. Massad. (b) These persons will be scheduled for a hands-on exam with the rest of the plant personnel. At the time of the hands-on Dr. Massad will obtain blood, urine and stool specimens, and medical/occupational histories. (c) Through arrangements with you, the samples obtained will be sent to Met-Path for analysis. The physical exam report and other results obtained at the local medical facility will be sent to Examinetics. It is our understanding that the laboratory charges for the tests specified will be billed according to direct Met-Path charges at the time, per your letter of November 17, 1980 to Dr. Massad. The additional costs of laboratory quality control ($2.50), merging physical exam data ($7.50) and computerizing the local medical facility results listed ($10.00) would be added. If this procedure is acceptable, please advise so that we may proceed for these missing exams at Lake Charles.
