Notice of Preliminary Findings Media: SDWA Facility Name: City of Liberal Public Water Supply Facility Address: 1401 East Pine Street, Liberal, KS 67901 EPA ID #: KS2017504 Date: 5/16/2025 This notice is provided to call your attention to the following preliminary findings regarding state and federal regulations. This notice does not constitute a compliance order and may not be a complete listing of all findings resulting from the inspection. Citation 1) SDWA 1433(a)(1)(A)(i)(vi) Description of Finding 1) Risk Resiliency Assessment (RRA) incomplete and did not include all required categories. 2) SDWA 1433(b)(1)- 2) (4) Emergency Response Plan (ERP) incomplete and did not include all required categories. 3) 40 CFR 141.86(a)(4): 3) Lead and Copper Sampling Pool/Plan did not include all available Tier 1 sites or sites with lead service lines. Current plan only listed 22 sites and requires 30 sites. Recommend adding additional sampling sites for the upcoming monitoring period of July - Sept 2025; 4) Wells: gaps in sanitary seals; well casing vents with holes in screens; well casing vents were used in the tubing used for draw- down tubes that were not downturned or extended 5) Well #45 casing vent with large mesh screen 6) Water tower roof access hatches (Roof and Dry) hatches did not have waterproof gaskets on access hatch doors. 7) Station (clear wells) #4, #3, #2 vents/overflow were equipped with large mesh (4 mesh) or had large holes in the mesh screen. 8) Station access hatch lids - no gaskets 9) Interior corrosion of Stations #2 and #3 piping and components 10) Light corrosion of Station #4 components in the chlorine feed building (exhaust system). - Replacing this year 11) Station access hatches were not equipped with waterproof gaskets on lids; 1 12) Station #1 no overflow or vent - built in 1940-50's no design criteria 13) Overflow screen on the Water Tower, was torn with large holes; 14) Erosion at the base of the Water Tower fill pipe due to replacing the manway; 15) Oil spills near well casings (natural gas-powered wells due to oil transfer from 55-gallon drums to pump's oil reservoir) - no secondary containment. - looking to replace with electric pumps instead of manual transfer pump. 16) Mouse droppings and mouse nest on well pump and in well house of Well #53 (nest removed at the time of the inspection, but droppings remained). - traps / poison - spoke with exterminator will send pictures of fix. 17) Well #53 - electrical junction box cover missing, and wire connections exposed - recommendation 18) Sanitary Seals on Wells # 52 and #11 gaps between well pump and pump pad 19) Well #63 out of service due to issues chlorine injector pump assembly and significant corrosion on chlorine gas cylinder assembly - Low chlorine residuals in 2024. 20) Well #62 - (submersible pump) missing conduit covering wires; 21) Well pump packing leaking - recommendation 22) Backflow prevention on hose connections to well casing lubrication lines during maintenance activities (power washing at Well #11; 23) Tree near Well #35 building base and exterior wall recommendation *Additional follow up with Building Department still needed on the tracking program/practices of backflow prevention device inspections conducted y the City. As a continuation of the inspection performed at your facility, you are asked to submit a written response within 14 calendar days of receipt of this notice. Your response should include a description of all 2 corrective actions taken and/or a schedule for completing the associated corrective action.
