Williams Subject: SAFETY AND ENVIRONMENTAL CHECKLIST FOR THE SOLUTION PLANT DISMANTLING The following Safety and Environmental Checklist is to be used as a guideline for the safety of personnel involved with the Solution Plant dismantling. This checklist has been revised from the proposed checklist of 12-26-78, JWC-109-78, by a final meeting held at the main lab conference room and attended by J. Gaines, C. Tanner, A. Andrews, M. Shepherd, J. Wilburn, J. Brewer and J. Coe. PRIOR TO WORK COMMENCING: 1. Soltex will provide any Soltex employee directly associated with the Solution Area dismantling with an x-ray and pulmonary function test prior to overseeing any work in the Solution Area. 2. The contractor (s) involved with the dismantling work will provide contract employees with an x-ray and a pulmonary function test prior to performing any work and give the company (Soltex) the results. 3. Soltex will provide an Environmental Consultant who trill provide the necessary air sampling equipment and * personnel to conduct air sampling and analysis to insure Soltex worker safety from an asbestos exposure. 4. The contractor(s) will provide the necessary air .sampling equipment and personnel to conduct air sampling and analysis to insure contractor worker safety from an asbestos exposure. JVJC-7-7 9 2 January 18, 197 5. Soltex will provide the necessary clothing, hand, eye and footwear to insure Soltex worker safety. 6j The contractor(s) will provide the necessary clothing, hand, eye and footwear to insure contract worker safety. 7. Disposable asbestos respirators (type 8710-example) will be provided by Soltex and the contractor(s). Supervision, consultants, or workers directly associated or in the near vicinity of the asbestos dismantling operation will wear an asbestos disposable respirator. 8. The Solution Area where asbestos dismantling is being accomplished will have signs posted as required which read: CAUTION ASBESTOS BREATHING ASBESTOS DUST MAY BE HAZARDOUS TO YOUR HEALTH 9. The Solution Area will be barricaded from the rest of the plant. 10. 'Caution* labels will be provided and affixed to all raw materials, mixtures, scrap, waste, debris, and other products containing asbestos fibers or to their containers. 11. Polyethylene bags provided by Soltex will be used for the asbestos waste containment which will then be placed (after affixing the asbestos label) in Conservation Services, Inc.'s containers. 12. Conservation Services, Inc. will provide the necessary containers on-site for the disposal of asbestos wastes from the Solution Area. JWC-7-79 3 January 18, 197 PROCEDURES DURING DISMANTLING: 1. Air sampling and monitoring will be accomplished and coordinated with Soltex and contract dismantling super vision to insure compliance and proper documentation. 2. Workers potentially involved or exposed to asbestos will be supplied necessary protective clothing and respirators. 3. All pipe cutting for removal will be accomplished by hand pipe cutters or pneumatic saws with water on the blade, during operations. No spark or flame producing equip ment other than what is listed will be allowed without the approval of: a. Soltex Operations consultant b. Contractor Superintendent/Foreman or Safety Supervisor c. Soltex Safety representative 4. Where applicable, vessels and columns will be lowered to ground level and removed to the approved area before stripping insulation. 5. At no time will asbestos be stripped from equipment at heights exceeding 50 feet. 6. Piping will be removed from racks and equipment and removed to the approved area before stripping. 7. Asbestos exposed during manual cutting and/or removal operations will be water soaked (wetted) then removed to the approved removal site. 8. Asbestos will be removed from all piping and equipment prior to the removal of same from property. 9. Asbestos will be maintained wet until sealed into leak tight bags and such bags deposited in a waste disposal container. 10\ Bags of wetted asbestos will be properly marked. 11. The disposal site will be properly permitted by Texas Department of Water Resources (TDWR). 12. Trip tickets on each load containing asbestos wastes will be handled by Art Aikey, "Waste Code #179390." JVC-7-79 4 13. Soltex employees requiring physicals are: Jin Gaines Carl Tanner C. D.
