PROCEDURE BULLETIN --TEXAS CITY PLAi SIGN AND BARRICADE PROCEDURES FOR OVERHEAD WORK P-0137 PLAINTIFF'S EXHIBIT I I. PURPOSE MON-1990 The purpose of this bulletin is to define the various safety measures which must be taken on all overhead work in the plant to assure safety to those at lower levels. II. SCOPE This procedure applies to all persons engaged in overhead work in the Texas City Plant including all contractors. The term "barricading" used in this bulletin shall mean com pletely encompassing the area directly under overhead work with barricade tape, wooden barricades or other means so as to prevent personnel from freely walking under the work. III. RESPONSIBILITY . The Monsanto supervisor responsible for the work execution will be accountable to see that the bulletin is enforced on overhead work. :v- IV. PROCEDURE K'i'i V ,v--V --r. A ' -/V'1'--'/' In order to afford the proper protection necessary and yet not place unreasonable burden upon the jobs, it is necessary to break the overhead work into the three categories below: A. Emergency or Short Duration Jobs (less than 4 hours) On overhead jobs which are emergency or short duration jobs, less than 4 hours, the safety measures needed to protect personnel and facilities shall be left to the discretion of the supervisor in charge of the work. The only exceptions to this are as follows: 1. When lifts are being made (manually or with hoisting devices) it is mandatory to post signs and barricades or post a ground man for constant observation during hoisting operations. 2. High personnel risk areas as noted in "C" below. (Good rule to follow is "If in doubt, barricade.") B. Overhead Work (4 hours or more) LAM006825 All overhead jobs which have a duration of 4 hours or more shall meet the following criteria: 005615 1. Barricades shall be positioned far enough away from impact area under work so as to prevent injury to personnel. 4. Supervisor in charge of work will be responsible for notifying Fire Chief and Security Supervision prior to posting any barricades in streets or accessways which may be needed during emergencies. C. High Personnel Exposure Areas Construction work over high personnel exposure areas, designated walkways, sidewalks, streets, doorways, etc. shall be barricaded and signs displayed no matter what the duration of the job.
