308 Information sheet Site A 1. I am the sole owner of the property with the following parcel (b) (6) , which matches the GPS coordinates as labeled site A. 2. Adam Galland 3. Adam Galland operated his own mini X and skid steer. at (b) (6) 4. Work started around May 2022 and ended around August 2022. No more work is being done in that area and no future work will be done. A detailed description of the work done around the NorthWest corner was sent previously. In addition the main head gate on channel 1 had some minor work done on it, such as old metal gate cut off, debris removed around the gate, and leak's around ditch patched or shored up. Both culverts were replaced where the road crosses channel 1 on the South end of the property. Those culverts have been in place for over 50 years. In regard to figure 3 exhibit C. A plastic culvert was placed in channel 1. When the Army Corps came and verified the delineation on the property they noticed the culvert and asked me to remove it or get file for a permit. I removed the culvert immediately. 5. All work was done on my behalf. 6. The work done on this property was simple maintenance and clean up work. No maps, drawings, or engineering were necessary. 7. A detailed description with pictures was sent previously. 8. No other on-site environmental assessments were done other than the ones you received from Civil Solutions. 9. No permits were issued because I wasn't aware of any permits that needed to be issued for the type of maintenance work being done. 10.I was not aware that I needed a permit to maintain my own property. When I was told I needed a permit for the culvert placed at exhibit C, I removed the culvert immediately, and therefor never issued for a permit. 11. There are no plans for any additional filling, excavating, or land disturbing. 12.You have a copy of my correspondence with Samual at the Army Corps. I do not have copies of that. 13.Civil solutions is the only one that did work regarding this area. You have copies of his delineation. 14.There are no other properties or entity on my behalf that has performed filling, dredging, or land disturbing activities. Site B 1. This question would be difficult to answer as I don't know of any known wetlands, streams, creeks, and other waterbodies that have been impacted by any work. With that said Witt Construction did improve the road coming into the property off of (b) (6) street. I'm not sure if any permits were required from Wasatch county to improve that dirt road. There was no written contract between me and Witt Construction. It was just a verbal agreement that they would improve the road. At the end of the road a culvert was put in west ditch which is a man made irrigation ditch. This work was done around May 2021 and was completed around September 2021. In November of 2022 around 200 trees were planted on the property. The trees were planet directly in the ground with no other soil added. No more is being done on that property. 5. All work was performed on my behalf. 6. No maps, drawings, or engineering was necessary for the work done. 7. The property looks very much the same as it did before the work commenced. Before the dirt road was improved it was just a 2 track path where farm trucks and vehicles would come in and out of the property. The property only had cotton tree's along the boarder of (b) (6) We placed around 200 trees that were to be planted around the property at some future date. 8. I am not aware of any official Army Corps on-site environmental assessment done on the property. 9. No permits were needed that I know of. Witt Construction may of gotten a permit from Wasatch County for the road improvement but I'm not sure. 10. I was not aware that any permits needed to be issued for the work being done. 11. There are no plans for any additional filling, excavating, or land disturbing. 12.There have been no written or verbal communication with any federal or state agency regarding this property on work done on this property. 13.There are no contracts with any agencies regarding any work done. 14.There are no other properties or entity on my behalf that has performed filling, dredging, or land disturbing activities.
