' This **2 year-old,'married voaan entered the employ of the Garlock- Company on May 20, 1918 and worked there steadily until July 3, 1928 in the textile room, operating a loom and a creel. She then returned to the same kind of work on September 17, 1928 and worked through June 29i 1929* She was then married and left the company's employ. She had three children. During her married life she la said to hare gained weight and to haTe had some shortness of breath* On December 3, 19^2 she again vent to work for the Garlook Company, in the same depart ment, and worked steadily until NoTeaber 2$, 19*0* this last date, she resigned "without any explanation"* .. On April 8, 19**3, *be vat seen by Dr. Karsh of Palmyra, H. X.; at this ' time she complained, of .pain in the ohest, shortness of breath and cough. On Kay 15, 19*0* her chest was x-rayed by. the State Department of Health Ciinlo at Newark, M* X. These films were examined by Drs* J. H. Green, S* G. Vhipple, John MacMillan and me. Dr. Green reports on then as follows: There is infiltration inrolYing the larger part of the right lung. The shadows are such denser near the hilum. There is a dense shadow in the left lung hear the hilua. The two dense shadows largely obscure the heart shadow. No accurate measurements of the heart can be given. The leares of the diaphragm are quite high. The right leaf of the diaphragm lies abore the 8th posterior rib and the left leaf lies Just below the 8th posterior rib,* There are some areas of decreased density in the lungs, suggesting emphysema". s On June 5f 19*0# she was again x-rayed by the State Department of Health, this time at Palmyra, N. X. This film shows "changes similar to those described in the film of Kay 15, 19*0* The dense shadows extending out near each.hilum obscure the heart shadow. There is in filtration involrlng the larger part of the right lung. The leaTes of the diaphragm are high". The diagnosis, made by the State Department of Health on the abore two film*, was "pulmonary fibrosis"* On or about September 26, 19&4, the patient was sent into the Genesee Hospital of Rochester, by Dr. Karsh; here, she vu attended by Dr. Yera Cleaons v. Garlook Company 2 Xartin 3. Lefcnen. On September 27, 1944, Another chest film via taken at the hospital. This shoved "infiltration of the lungs, *3 described in the tvo previous films. In Addition, there la a pn e-um otharax pocket in the right pleural apace, extending along the axillary border and orer the apex, causing approximately 25% cr 30^ compression of the lung. The heart is apparently .shirted slightly toward the left side. In addition to the infiltration described in both lungs in the previous films, there la a alight increase in the linear narklngs in the left lung, from the clavicle to the base". The patient died at the hospital on September 29 1944,. . .* Autopsy vaa dene by Dr. K, V. Pcpcff, the hospital pathologist. He will forward a complete report of his findings, both gross arsd micro scopic* The autopsy material vas reviewed by Drs, Vhipple, KacXillan and as. It establishes the conclusion that the patient died.1** a reault of pneumothorax. This pne^notharax vaa associated vith extensive pulmonary fibrosia, vith esphyseuaatous blebs and esphyseaa. Sections of the lungs confirm the diagnosis of pulmonary fibrosis, resulting fron prolonged contact vith asbestos1. It is Dr. Pcpoff's opinion that the asbsstosis vas "of long duration*. The advisability of aaiing.a chemical examination of the ashed lung vas taken up vith Dr. Leroy U. Gardner of the Edvard L. Trudeau founda tion at Saranac Lake, N. I, He vrote me: *1 would auch rather, rely, on a histological examination of large blocks of lung tissue aade by a ' competent pathologist than I vould expect results from chemical data. At best, the latter is only supporting evidence of any morphological ohangee that may be present. Ve have done it on a good many lungs that hare proved to have asbestoeis, but all one can do is to demon strate that there nay be abnormal quantities of nagnesits*. The sili cates that,are detected night readily be due to other Inhaled materials* Cn the basis cf the above, it vas not thought necessary to hare the lung ashed. . Conclusion! It is cay opinion, in which Dr*. Popoff, XaoXillaa and" vhipple tell ne they concur, that this voaan died of ajneunothorax, associated vith ptimenary fibrosis and asbvvtdsis; that this process began at some tine after she first vent to vork for the Oarlock Packing Company and steadily progressed; that she vas, clinically-, an advanced case cf asbeatosis vh*n she returned to work for the Garloek Company in December 1942, and that the proems* vould undoubtedly have gone cn to a fatal termination even if she had not worked during the period from December 1942 to Her amber 1943-.
