The inspection began at approximately 1:00 p.m. on May 21, 2025, and finished at approximately 3:15 p.m. on May 21, 2025. We met Tricia Freytag, Manager/Operator and Jennifer Vicha, Office Clerk. The team presented their credentials to the System representatives, and informed them that this was an EPA inspection to determine the water system facility's compliance under the Safe Drinking Water Act, and it was a part of an arsenic rule training exercise for federal drinking water inspectors. The scope of the inspection was to inspect the physical assets, treatment, and operations and management of the water system. The inspection was not a full Sanitary Survey, nor did it take the place of the Texas Commission on Environmental Quality (TCEQ) Comprehensive Compliance Investigation (CCI). At the time of inspection, the EPA inspection team visited the following elements of the Axtell WSC System: Axtell WSC Plant 1 o Well No.1 o Chlorine room o Cooling tower o Service pumps o Ground storage tanks o Elevated storage tank o Generator Axtell WSC Plant 2 o Well No.2 o Generator Photos taken during the inspection are included in Appendix 1. FACILITY DESCRIPTION Axtell WSC Public Water system (PWS) is a groundwater system that provides service to 638 connections and an estimated population of 1,914 individual, according to the Safe Drinking Water Information System (SDWIS) Database. The Axtell WSC is classified as a community water system. The system is a single pressure plane water system supplied by two wells. The Trinity Aquifer serves as the water source for the system's wells. The Axtell WSC consists of two plants. Plant 1 is located at 207 5th Street in Axtell, and includes Well No.1, a cooling tower, a chlorine room, an elevated storage tank with capacity of 0.2 million gallons (MG), two service pumps capable of 500 gallons per minute (GPM) each, a generator, and two ground storage tanks of 0.1 MG. Plant 2 consists solely of Well No. 2 and a generator, and is located approximately 0.75 miles north of Plant 1 on Riley Road. The Axtell WSC has an emergency interconnect 2 Axtell WSC Inspection Date 05/21/2025 with EOL WSC. The interconnection is located at the junction of Highway 84 and Farm-to-Market Road 1330. In addition, according to the system representative, the system submitted lead service line inventory (LSLI) to TCEQ. Based on the LSLI, the Axtell WSC doesn't have a lead service line. Water is delivered from Well No. 1 at a rate of 200 GPM and from Well No. 2 at a rate of 350 GPM. Water from both wells is combined and disinfected with chlorine before reaching the cooling tower. Within the cooling tower, the water temperature is adjusted, and the water is then transmitted to a 0.1 MG ground storage tank. Two service pumps, each with a capacity of 500 GPM, transfer the water from the ground storage tank to a 0.2 MG elevated storage tank. The water stored in the elevated tank is subsequently delivered to the distribution system. STAFFING INFORMATION The system is operated twenty-four hours per day, seven days per week (24/7) with two operational personnel. Tricia, the manager/operator of Axtell WSC, holds a Class D Groundwater operator certificate and primarily operates the system. Johnny Childers, another operator at Axtell WSC, also possesses a Class D Groundwater operator certificate. He is responsible for checking the water meter daily and managing the system's landscaping. Jennifer, the office clerk, handles administrative and business work while supporting Tricia. She is working towards obtaining her Class D Groundwater operator certificate. Mark Britton, a contractor at Axtell WSC, assists with operations such as leaking issues, meter reading, and the delivery of diesel fuel for emergencies. ARSENIC ISSUE DISCUSSION The Axtell WSC has been experiencing violations of the arsenic maximum contaminant level (MCL) (> 0.01 mg/L) for over 10 years due to its water source, the Trinity Aquifer, which contains high arsenic concentrations and elevated water temperatures (130F). This arsenic issue is not unique to the Axtell WSC; other small groundwater systems using the Trinity Aquifer also face similar challenges. To address this problem, local public water systems (PWSs) in Falls, Hill, Limestone, and Mclennan (FHLM) counties have come together to establish the FHLM Regional Water Corporation. The Axtell WSC is a member of the regional water planning group. One of the initiatives to reduce arsenic levels involves a collaboration between the FHLM Regional Water Corporation and the City of Waco. This project aims to blend treated surface water from the City of Waco with the Axtell WSC's water supply to lower the arsenic concentration. Currently, the system is engaged in easement negotiations for the placement of a pipeline necessary for this blending project. Section II-OBSERVATIONS During the inspection, we inspected the water system's Wells (No.land No.2), a chlorine room, a cooling tower, two ground storage tanks, two service pumps, an elevated storage tank, and two generators as well as document review. Axtell WSC Plant 1 Well No.1 o We observed Well No.l equipped with 200 GPM pump and the pipe from the well covered with the insulation materials at the time of inspection (Photo 1). 3 Axtell WSC Inspection Date 05/21/2025 o An open conduit was observed in the wellhead of Well No. 1 (Photo 2; AOC 1). Chlorine room o We observed a chlorine room (Photo 3) with a door that displayed a chlorine warning sign and was equipped with a lock. At the time of inspection, chlorine cylinders were not chained (Photo 4; AOC 2). o The chlorine room was equipped with a ventilation system (Photo 5; AOC 3). The ventilation system operated manually, with its switch located on the outside wall of the chlorine room. Cooling tower o We observed a cooling tower (Photo 6) and discussed the geothermal and arsenic issues in the Trinity Aquifer. According to system representative, the average water temperature from their wells is approximately 130F. Ground Storage Tank o Two 0.1 MG ground storage tanks were observed to be in good condition (Photo 7; Photo 8). One tank was active, while the other remained inactive. The inactive tank is planned to be used for mixing the system's water with treated water from a nearby public water system to reduce the arsenic concentration in the system's water. Threaded sample tap on the active tank didn't seem to have backflow prevention device. (Photo 9; AOC 4) Service Pumps o We observed two service pumps with capacity 500 GPM each housed in the building (Photo 10). o We observed that the drain valve was equipped with backflow prevention device (Photo 11). Elevated Storage Tank o We observed a 0.2 MG elevated storage tank (Photo 8). The tank appears to have been built in 2010, and the height to the high level is 180 feet. Threaded sample tap on the elevated storage tank didn't seem to have backflow prevention device. (Photo 11; AOC 4) Generator o We observed that a generator was installed on-site for emergency situations (Photo 8). The generator operates on diesel fuel, which is not stored on-site. Instead, Mark Britton, a contractor at Axtell WSC, delivers the fuel for the generator as needed. Fence o We observed a fence surrounding the plant, which appears to be properly installed. However, there are some areas with gaps under the fencing due to erosion (Photo 13; AOC 7). Axtell WSC Plant 2 Well No.2 o We observed Well No.2 equipped with 350 GPM pump and the pipe from the well covered with the insulation materials at the time of inspection (Photo 14). o An opening conduit was observed in the wellhead of Well No. 2 (Photo 15; AOC 2). o We observed the ripped mesh on the groundwater vent (Photo 16; AOC 8). o We observed missing screen on the air release discharge line in the wellhead of Well No.2 (Photo 17; AOC 5) 4 Axtell WSC Inspection Date 05/21/2025 o We observed temporary pipe supports being used under delivery line. {Photo 18; AOC 6). Generator o A generator for emergency use was observed in Plant 2 {Photo 19). The generator in Plant 2 is operated by diesel fuel such as the generator in Plant 1. According to the system representative, the diesel fuel is not stored on-site and is delivered by the contractor as needed. Fence o We observed a fence surrounding the plant, which appears to be properly installed. However, we also found gaps under the fencing caused by erosion {Photo 20; AOC 7). Operation and Management {O&M) Document review o We observed a document including O&M information such as utility operation major components & processes, drought contingency and emergency water demand management plan, startup procedures, normal operating procedures, and maintenance check list. o We observed line flushing procedure and flushing log. According to the system representative, it is regularly conducted once month. o We observed records for daily meter readings and biological sample results. Section Ill - AREAS OF CONCERN (AOC) AOC No. 1. AOC No. 2. AOC No. 3. AOC No. 4. An open conduit was observed at the wellhead of both Well No. 1 and Well No. 2. We recommend properly sealing or blocking the open conduit in both wells to prevent potential contamination and ensure the integrity of the water supply system. The chlorine room lacked several essential safety features, including a crash bar on the door, a window, sensors, and alarms for detecting chlorine leaks. Additionally, the chlorine cylinders were not secured with chains. We recommend securing the chlorine cylinders with chains and installing the necessary safety features. Chaining the cylinders can help store the tanks safely and prevent potential leaks. A crash bar on the door will facilitate a quick evacuation in case of a chlorine leak, while sensors and alarms will ensure prompt detection of gas leaks within the room. Ventilation system was operated manually with the switch on the outside wall of the room. We recommend configuring the system to activate automatically when the door is opened, as this provides a better practice for managing potential chlorine leaks. The threaded sample taps on both 0.1 MG active ground tank and 0.2 MG elevated storage tank did not appear to have backflow prevention devices. We recommend installing backflow prevention devices on the threaded sample taps to prevent potential contamination. 5 Axtell WSC Inspection Date 05/21/2025 AOC No. 5. AOC No. 6. AOC No. 7. AOC No. 8. A missing screen was observed on the air release discharge line in the wellhead of Well No. 2. We recommend installing a screen on the air release discharge line to prevent potential contamination. Temporary pipe supports were observed under the delivery line. We recommend installing permanent, structurally stable supports for the pipes. This will enhance structural stability and can increase the longevity of the pipe. Gap under the fencing was found in Plant 1 and 2. We recommend eliminating the gap in both plants to enhance security and prevent unauthorized access or intrusion. A mesh on the groundwater vent was found to be ripped in Well No. 2. We recommend replacing the mesh on the vents of the well to ensure proper protection and functionality. EPA inspection team conducted a closing conference at the Axtell WSC with Tricia Freytag and Jennifer Vicha at 3:07 PM on May 21, 2025, for the inspection. During the closing conference, the inspection team reviewed the eight (8) Areas of Concern {AOC) noted during the inspection and our recommendations. Section IV - FOLLOW UP The following information was received by EPA on June 27 and July 3, 2025, after exiting the Facility on May 21 {Photos taken during the inspection are included in Appendix 4, Corrective Actions): An open conduit was sealed at the wellhead of both Well No. 1 and Well No. 2 {AOC No. 1). The system installed chlorine sensor, and the chlorine cylinders were secured with chains {AOC No. 2). New screen was installed on the air release discharge line in the wellhead of Well No. 2 {AOC No. 5). Gaps under the fencing in Plant 1 and 2 were filled to enhance security and prevent unauthorized access or intrusion (AOC No. 7). Ripped mesh on the vent of Well No. 2 was replaced (AOC No. 8).
