Reports The Monthly Safety and Housekeeping Audits land Injury Summary for January were reviewed: SOC and operating range markings on gages continue to be found in poor repair. The need still exists for this job to be done. Storage of aerosols and other flammables; continues to be a problem. Jim Borden will develop some sitorage guidelines. Oil on grating and around pump and compressor bases presents slipping hazards. ' Conduits lacking seals continue to be found. Lack of adherence to the seat belt wearing requirement was noted. Several instances of inadequate personnel burn protection were noted. The number of items needing correction was deemed excessive. On the positive side.were two areas with no alarm lights in the alarm condition. Failure to yield the right of way to cranes was noted, it is suggested that groups review S F 67. II. Monthly Injury Summary There were three high potential injuries, ail covered by Incident Reports. ! Congratulations to Alathon Lab, HMD Maintenance Shop, "J" Unit Reaction, Operation 4, 524 Bldg, 513 509 T/F as Good House keeping Award winners. III. Incident Reports Reports 78-77 through 79-3 were reviewed: ! 78-77 was a result of operating out of the established operating range. Concern was expressed at five pumps being noted with out seal flush a month after this fire occurred due to lack of seal flush. 78-78 - Information gathered has been transmitted to Sabine as a result of their PuLl Rod Failure five days after the one at Victoria. DUP 0801447 DU 007914 Victoria Plant Central Safety Committee Minutes of Meeting Held February 12, 1979 1 i Page 3 78-79 was regarded as a positive report.: 78-80 - Good patrol and observation by the operator detecting the leak. 78-82 - Recommendation No. 4 was a result of the switchgear door being opened and the fire extinguis'her used without the use of personal protective equipment. 79-1 - The committee formed to prepare freeze precaution guide lines will report to Central Safety in March. 79-2 - Improper positioning and not using the best tool for the job. Emphasize training of people to use the best tool available for the job. 79-3 - The man did not assure himself that the system was clear. Another problem was the loss of communications across shift change. ,, ! i. IV. Safety Activities Committee - W. T. Dodson : On Plant : No lost workday injuries, 438 safe days.; One restricted workday injury on January'14th - slip on ice. Achieved first 1979 goal - 3MM safe hours on February 7th. Next goal of 4MM safe hours about June 29th. On Plant Activities Steering Committee and Subcommittee staffed. Detailed program formulation in progress: Lionel Getz, Mechanical Subcommittee Chairman, gave an over view of the planned programs. It was suggested that CPR train ing be considered for all E I personnel. Off Plant No off-plant injuries year-to-date. Achieved first 1979 goal - 50 safe days on February 9th. Next goal - 100 safe-days on March 31st.: DDC Instructor Training set up for February 21-22. Anticipate beginning instruction in mid-March. \ DUP 0801448 DU 007915 Victoria Plant Central Safety Committee Minutes of Meeting Held February 12, 1979 Page 4 John Chambard, Auto Safety Subcommittee Chairman, outlined the various auto safety programs that have;been planned. The list of the 1979 On and Off-Plant Safety Subcommittees Chairmen and members is attached. V. Eye Wash Facilities - M. D. Smith Mike made the following recommendations: Include note on minimum flow in VPS 8E I Upgrade all stations to comply with 8E, 3-3 (Inspect but do not test during freeze precautions) Change design to improve draining by drilling a 1/16" hole in piping near flexible hose inlet i Remove all clips previously used to hold the spray heads in the up position Revise VPS 8E to require a drainage hole; in the handle VI. Contamination Monitoring of Acid Suits - Dr'. Culpepper Dr. Culpepper pointed out the need to monitor acid suits for contamination before each use and/or eliminate areas (straps) that are absorbent. Proprietors will review acid suit practices. VII. Kirk-Key System - R. D. Oakley , Bob gave a description on the use of a Kirk-Key system, its advantages and safeguards. Approval was' given for the develop ment of a plant standard to govern the Kirkj-Key use. To be included in the standard is a requirement f'or a gate position indicator. - i VIII. S F Procedures Nos. 3, 20 and Work Permit! Form - C. E. Bunn Changes in S F Nos. 3 and 20 were approved and will be issued promptly. The revised work permit form was: approved. An order has been placed for the new forms. IX. Key Safety Data j The Incident Frequency Rate of 5.69 for! January is higher than the 1978 average and is not good performance. The warning flags are up, and the Chairman reiterated that safety is still number one on this plant.
