Answers to numbered questions in your memo entitled ASBESTOS INSULATION IN 2-4-8L.
(1) The group doing the work (ie Linde Support Services supervised by W. C. Seelig) because the supervisor who signed the upper right blank on the HWP Permit also closes out the work by signing the lower right blank marked, signifying that the "Work has been satisfactorily completed and area inspected". The problem in this case is that I signed the permit without specifying the disposal procedure/responsibility. Bob McCarroll apparently needs such a reminder. In the light of all this please seal the bags, and consult Gary Small (ext. 3172) as to what container to put them in so they don't get punctured. Then have Material Handling pick it up and get them to Gary. I know you and JMS know how to rate the hazard and tag the bags.
(2) The site Industrial Hygienist said no. I agreed because the worker was using all Methods of compliance outlined in OSHA Standard 1910.1001, Section (c). The air-purifying respirator (vs powered air-purifying, or air-line supplied types) met item(d) of this standard, because the 8 Hour Time Weighted Average was "not reasonably expected to exceed no more that 10 times either, the PEL of 2 fibers more than 5 micrometers long per cc of air in the work area averaged over 8 hours, or the ceiling limit of 10 fibers more than 5 micrometers/cc for any period of time. This means less than 20 to 100 fibers per cc of air outside the respirator.
(3) Absolutely, this is UCC policy. OSHA assures that a worker can see these results in his Health file as a minimum standard. Jim should see the results, especially because they were taken outside the enclosure of Bob McCarroll's work area (ie Jim's work area) .
(4) Frank Lutz (the site Industrial Hygienist) is responsible for all such analytical work. Just give him a charge number.
(5) Ask Frank Lutz to monitor your lab regularly from now on. Just request it in writing, with frequency, and a charge number.
(6) Not unless you have evidence that the PEL (2 fibers/CC over 8 Hours) is exceeded, or your manager agrees to pay the con tractor without such evidence.

(7) Probably it is, unless the insulation is crumbling and is known to be mainly asbestos. Encapsulation avoids creating an aerosol from cutting and handling insulation which is not
