Some time ago I sent you a letter suggesting you budget this work a three-year period at the rate of $7,000 a year, the funds to be de available as soon as Clinton gains final release from hiB present duties, which can be at an early date* if we want this work to start ismediately. My brother and I have felt we would pay half of Clinton's salary - $2,250 a year - from funds supplied by you,, and would attempt
to get a thoroughly well-Qualified assistant for about $2i`&00 to $3 000. This assistant must have a good knowledge of organic chemistry
together with a certain amount of physical chemistry. Ee, or she. will have ample opportunity to supplement this knowledge by whatever attention he wishes to give to pharmao&ogy and biochemistry. This allotment of funds will leave us a margin which we will be able to use for various purposes in getting special information, and I am certain that after the first year there will be experimental work that must be carried out and which will, of course cost money Furthermore, we may expect that Dr. Clinton will need to do a certain amount of traveling in order to acquaint himself thoroughly with all of your needs, and for this we should have the margin in our funds which will be secured by the disposition I have mentioned*
We shall make Dr* Clinton an Associate in my brother's department of Industrial Hygiene and he will, of course, devote tome of hie time to familiarizing himself with new techniques, etc,, in my brother's group. This report sunnarizes the objectives and the current activities oi
'our group at Harvard in connection with our contract with the American Petroleum
Institute. Suggestions for extending the program are included
The principal activity of our group at present is the compilation and formulation of toxicological reviews based on the available literature concern ing a variety of compounds of interest to the industry The available information on each compound is being prepared in outline form for submission to the Medical Advisory Committee, more or less according to the outline suggested by the Subcommittee on Permissible Concentrations. It is now our practice to present each review to the Medical Advisory Cocmittee for comments, criticisms and suggestions before preparing final drafts. This enables us to benefit from the invaluable practical experience of the medical and technical departments within the industry. Their response so far has been most gratifying. It is realized that compiling a separate outline for each substance frequently results in repetition, particularly in the lections dealing with placement e;aminations and safety precautions. Such repetition appears justifiable, however, if the outline of information on each substance is to be a complete unit which can be referred to rapidly and without reference to other substances. It is our belief that this should be continued.
The work of compiling information does not progress with great
rapidity, for we consider it best to concentrate on quality rather than
quantity. This necessitates repeated searching, checking and double checking,
all of which are time-consuming. So far, tentative drafts of information have
been sent to Mr. Stroop for distribution to the Medical Advisory Committee on
the following substances:

It is noted that three of the eleven companies do not keep all cords of sickness and accidents in the medical department and then have the ^formation available only through doctors and nurses as is done in but six of
even companies, Your subcomittee strongly reco:amends that all medical ^cords be kept by the medical department. It is evident from the survey that too many higher ups are entitled to the information in the records. It is Vjoar.ended that every effort to avoid acquainting superiors vdth the physical Editions recorded in the medical records be kept to a minimum. However, it ?. rccognized that they are entitled to information regarding the degree of disability and the cause of disability only when such disability results from viorlcing conditions.
A definite rehabilitation plan is essential in the opinion of your subcommittee which plan can well include the Veterans. It is to be noted that seven of the companies have a definite plan for the Veterans, but it is the opinion of the subcommittee that the care and placement of the Veterans is not the problem that was anticipated; therefore, it can well be included in a general plan.
* * Health education in industry should be a featured practice of the aedical departments but from the answers in the questionnaires on this activity it appears that this phase of the program is sadly neglected. Your subcommittee! therefore, recommends a definite health education program be conducted continually.
The questionnaire shows that but five of the companies have aedical inspection of working conditions. Your subcommittee wishes to reconmend this procedure as necessary for conducting a good health program, as economically sound in preventing sickness and accidents as well as improving working conditions and personal contacts.
The medical departments of the companies reporting apparently are
not interested in research. Industrial medicine isTnostly practical medicine,
therefore, it is recognized that this subject is better left to collective
action and institutions adequately equipped for research.

How physicians in industry are to keep posted on proposed legislation and legal matters is a problem not answered by the questionnaire. The study of this subject is recommended by your subcommittee for the purpose of developing a satisfactory policy of keeping the doctors informed on this subject.
Although the questionnaire does not indicate much activity on the
part of the doctors in connunity health, nevertheless it is believed that they
are doing a good job as they recognize that employees spend ^two-thirds of their
time in the community.

The report of the meeting of the Subcommittee on Carcinogenicity held '(arch 14 1947 was discussed. Photostat copies of three articles concerning ^ carcinogenic properties of petroleum products that have appeared in "The ^ crican Y/ecldy" "Life," and "Medicine" were presented and discussed.
The report of the Subcommittee on Carcinogenicity was presented and is given in full in Appendix A.
Dr. F. F. Heyroth of the Kettering laboratory of Applied Physiology,
University of Cincinnati School of Medicine, presented a progress report on the
oxnerincntal carcinogenic studies that they are carrying out on several petroleum
samples for the API. A letter from Dr. Heyroth to Dr. Woody, dated April 25, 1947 describing the results of the studies that are being carried out at the University
0f Cincinnati is given in Appendix B.

Iu receiving the Subcommittee <s report, the Mcdi'-'.i Advisory Committee voted to adopt recommendations as follows:
* V A. The animal experiments now under way at the Kettering Laboratory
of Applied Physiology should be continued at least until the end of the current yoar.
B. Extend the scope of investigation to a largo number of samples so
I that they can be treated on a statistical basis rather than on a comparative basis using a small number of samples as in the present work. It would seem that the best attack would bo to get out a form letter to a number of oil companies inquiring whether or not they would be willing to participate in such a program and if so, request them to set aside barrel samples of catalytic recycle stock, catalytic resids, thermal recycle, hydroformer bottoms, thermal resids and other samples,,and advise by what type of operation and under what conditions they have been producod. Then, after an analysis of responses to that questionnaire as many of these samples will be studied as is practical.
C. The Chairman of the Subcommittee on Carcinogenicity is authorized to rcloasc samples up to one gallon of each, and to a total of 25 gallons, for physical and chemical studies with the objective of correlating these data with the tests on mice. The results of these physical and chemical studies shall be submitted to the Subcommittee on Carcinogenicity.
D. Establish a registry of malignancies to report to the Subcommittee on Carcinogenicity. This registry would collect data on all malignancies incident in employees of the ur roleum industry. It would set up its own procedures, forms, cxtc.it of data, follow-ups, pathology, treatment, outcofao, autopsy reports, etc. after consultations with existing registries.
E. Means must be found to refute the false and misleading statements that arc continually appearing in newspapers and magazines as to the prevalence of cancer among those who work in refineries and

(1) Initiate a literature survey on the possible carcinogenic effects of petroleum products to nan.
(2) Initiate a study of the physical and^chcmical characteristics of petroleum products in an attempt to obtain a simple procedure to determine whether a particular petroleum sample is or is not carcinogenic.
(3) To have experimental studies carried out on sludge, lubricating oils, gasoline and other oils.

pfeired with the medical directors of several oil companies to see what l-dical records are available and to determine what epidemiological techniques can he used to advantage. The long incubation period of cancer is one of the factors peculiar to the problem. A cancer registry for the petroleum industry
one of Dr. Phair's objectives, but he has his doubts concerning the success ful enforcement of laws that require the reporting of occupational cancer as advocated by some authorities.
The National Cancer Conference which was held in Memphis, February 25-27 inclusive, under the sponsorship of the American Cancer Society und the National Cancer Institute of the U.S. Public Health Service was attended by over 350 investigatore from all parts of the country, of whom gU were assigned to the panel on occupational cancer. It was quite evident that the problem of carcinogenic agents in the petroleum industry is an open booh and that control measures, both medical and technical, must be adopted, jt was also agreed that the petroleum industry is to be congratulated on having undertaken a comprehensive study of the pressing problems.
A similar situation has developed recently in England, where the Medical Research Council has appointed a committee on occupational cancer of eight members, including Colonel S. J. M. Auld representing the Institute of petroleum. He informs us that certain lubricating oils, notably those used in the textile industry, are being looked upon with suspicion and that some companies have undertaken the manufacture of white oils as substitutes, although there is no demand for them as yet. The valuable research done at Birmingham is now going forward with government support, and in view of the similarity of the problem in both countries, Dr. Auld advocated a free exchange of information between the American Petroleum Institute and the Institute of Petroleum.
It is recommended that:
1. The American Petroleum Institute exchange information on cancer with the Institute of Petroleum.
2. The allocation of funds to continue cancer research at the University of Cincinnati for the 12 months ending June 30, 1950 be increased from $50,000 to $100,000.

A fairly detailed review on the status of Occupational Cancer has been prepared and distributed under the joint auspices of the Subcommittee on Carcinogenesis and Permissible Concentrations. A detailed, comprehensive review on Dermatitis due to Cutting Fluids wae prepared under the auspices of the Subcoonittee on Cutting Fluids and sent to Dr. Reeve for approval. At Dr. Reeve'a request, etone lie are now being prepared on this review. Tentative drafts on phenyl naphthylaalne and
U.O.P. #5 were also distributed.
At present, literature survey information has been collected, but it has not been put into final form for distribution to the Medical Advisory Ccoalttee. This information consists of a number of substances. The question
arisen ae to whether these are of sufficient interest to justify the effort required to complete them, and whether any further literature survey work should

X. (a) We submit the list of compounds given in the attached letter from

toxicological reviews have been prepared, for final approval and
referral to the Editorial Committee.
(b) A number of last minute changes concerning the above revlevs have been taken into consideration by Dr. Clinton and have been Incorporated in the final reviews that will be submitted to the Editorial Comal ttee.
2. The work on the toxicological revlevs will be continued at Harvard under the direction of Professor Philip Drinker on the remaining compounds given on the original list that wae approved by the Medical Advisory Committee.
Professor Drinker has agreed to provide readily available information on particular substances at the request of members or associate members of the API Medical Advisory Committee. Copies of such inquiries and replies should be sent to Mr. Stroop for distribution to all members of the conmittee.
3. A new list of tentative maximum allowable concentrations for various substances is being prepared by the Association of Governmental Industrial Hygienists for conslderatlon-et their Spring meeting in Boston. Many of these substances are of Interest to the petroleum industry.
It is Professor Drinker's opinion that some of these proposed 11mlts are unreasonably low.
Professor Drinker has offered to represent the API Medical Advisory Committee at the public meeting of this group. He proposes to circularize the Medical Advisory Consolttee stating which " T i m m allowable concentrations appear to be unreasonable, givix^j the evidence to support his views. If any member of the committee is ii disagreement with Professor Drinker's views, and he expresses this by letter to Professor Drinker, no action will be taken on that particular substance.
U. The committee recommends that the designation, "API Toxicological Reviews" be retained.
5. It is recemended that $10,000 be appropriated to Harvard University to
continue the wbrk of Professor Drinker and associates for the period,
April 1, 19U8 to April 1, 191*9. Professor Drinker plans to continue
preparation of Toxicological Revlevs and to initiate the screening test
program approved at the canal ttee meeting in April, 191*7 .

At its meeting in Boston on April 1, 19^8, your subconsaittee voted to yeport information and recommendations to the Medical Advisory Committee as follows:

Cooperation with the Medical Advisory Committee of the Manufacturing
Chemists Association.
In conference with Doctors J. M. Carlisle, A. G. Cranch and D. 0. Hamblin, representing the M.C.A. Medical Advisory Committee, Dr. Cranch expressed the opinion that there was no objection to the issuance of API Toxicological Reviews on substances which are or may be included in tho Chemical Safety Data Sheets of the Manufacturing Chemists Association oven though there might he inconsistent statements in such publications.
Following a discussion of ways and means to facilitate tho exchange of
information during the formulation of API Toxicological Reviews it was agreed that the members of the MCA Medical Advisory Committee should be made consulting members of the Subcommittee on Permissible Concentrations of the API Medical
Advisory Committee.
Work at Harvard University School of Public Hoalth.
A. M.A.C. Values for 19U8.
Professor Drinkor reported a numbor of-shanges^adopted by the
committee of government hygienists for publication in the I9W
M.A.C. Values. He stated that the now values will be made available
promptly to all members of the API Committee.
B. Basis for Future Changes in M.A.C. Values.
Professor .Drinker stated there was definite need for tho gathering and publication of quantitative data which would contradict the current theoretical M.A.C. Values on many substances.
C. Report on Status of Harvard Work.
Tho subcommittee reviewed and accepted the report dated March 10th from Professor Drinker to Dr. Baird (b o o copy attached).
D. Toxicological Reviews.
The subcommittee recommends that no toxicological reviews should be published on substances for which literature surveys by Professor Drinker and associates rovoal little or no available toxicological information, but instead that every six months

Company A reports that 98.5 per cent of the catalyst leaving the
stack is less than 5 microns in size and company B states that "Most of the
catalyst lost from catalytic cracking operations is estimated to he less than
10 microns."
We nave no definite information concerning the silicotic potency of the catalyst dust hut even if we were to assume that it was as toxic as crystalline silica of equivalent particle size, there would he no hazard to the neighborhood from dust concentrations of the magnitude tabulated above.
During certain operations involving handling of the catalyst,

workers in company A were exposed to concentrations as high as 120 million
particles per cubic foot. Such concentrations, even though exposure is limited
are potentially hazardous and the employees should wear suitable respirators.
Some members of the Medical Advisory Committee have reported that they have no dust counts available and have requested our opinion as to the
necessity for obtaining them. "That the Safety Committee of the Board of Directors of the API be requested to ask the Medical Advisory Committee to refer to the General Committee of the Division of Refining all material dealing with or affecting refinery operation or petroleum products, prior to approval for the publication of this material by the Institute or through other channels."
Statements made by the members Indicated that the law of diminishing
returns was in operation on the Toxicological Review work carried on at Harvard
University.
Action
On motion, the Committee voted to terminate the Toxicological
Review project at Harvard University as of June 30, 1950, and
to recommend the payment of $3,000 to cover the period from
i March lU, 1950 to June 30, 1950.
The Committee also voted to reconsnend that Professor Philip
1 Drinker should be retained by the Medical Advisory Committee
as a consultant in industrial hygiene for the year ending

Toxicological reviews on alkyl nitrates, solvent naphthas, and triethanolamine are In an advanced stage of preparation and tentative drafts will be ready for distribu tion in the near future. A search was made for toxicological data on amino phenols and sulfurized olefins but no significant data were found. Drafts of toxicological reviews of U.O.P. #5, asmonium molybdate, and asmonium nitrate were revised to Incorporate suggestions made by members of the Medical Advisory Committee.

As stated In the report presented at the April 19^9 meeting, the initial experiment to determine the silicotic potency of fresh and spent silica-alumina catalyst by the intraperltoneil technique In guinea pigs was unsatisfactory because the animals died of an infoctlon early in the experi ment. A second experiment was begun in March 19^9 in the hope that the animals would survive for at least nine months. We have been fortunate in this experiment as the majority of the animals are still living. They be sacrificed in December 19^9
b. Survey of Neighborhood Exposure to Catalyst Dust
While we have had several inquiries from members of the Medical Advisory Ccenlttee during the past six months concerning survey methods, no additional data have been submitted on dust concentrations in the neighborhood of refineries using silica-alumina catalyst.
Proposed Future Work
A questionnaire was distributed in April 19^9 listing 75 compound for which toxicological information had beem requested by members of the Mb d - m u Advisory Committee, asklxg that each member indicate the 5 compounds which he wished to be given priority In preparation of reviews. Conrpany A reports that 98.5 per cent of the catalyst leaving the stack Is less than 5 microns In size and company B states that "Most of the catalyst lost from catalytic cracking operations Is estimated to he less than
10 microns."
We nave no definite information concerning the silicotic potency of the catalyst dust hut even if ve vere to assume that it was as toxic as crystalline silica of equivalent particle size, there would he no hazard to the neighborhood from dust concentrations of the magnitude tabulated above.
During certain operations involving handling of the catalyst, workers in company A were exposed to concentrations as high as 120 million particles per cubic foot. Such concentrations, even, though exposure is limited are potentially hazardous and the employees should wear suitable respirators.
Some members of the Medical Advisory Committee have reported that they have no dust counts available and have requested our opinion as to the necessity for obtaining them. I am submitting herewith a revised Progress Report for the Proposed Epidemiological Studies of the Project on Carcinogenicity at the Kettering Laboratory. It contains the modifications and revisions suggested for the report presented at the meeting of tne Subcommittee on Carcinogenicity of the Medical Advisory Committee h eld April 23, 1950 in Chicago.
The standard clinical record and occupational h i s t o r y forms to be employed in gathering the information required for the Retrospective Study and the Current Registry are in prepar a t i o n , a n d it is h o p e d that these can be d i s t r i b u t e d for immediate u se e a rly in June. It is p l a n n e d that the code to be used in the tabulation of these data will be ready for circulation at the same time. As many personal vi s i t s as f e a s i b l e w i l l b e m a d e b e f o r e th e N o v e m b e r m e e t i n g of the Subcommittee, so that a uniform handling of these records can be insured, insofar as possible, u n d e r e x isting conditions.
As an additional activity of the epidemiological survey, a simple questionnaire requesting certain fundamental d e s c r i p t i v e fac.ts a b o u t e a c h r e f i n e r y o r i n s t a l l a t i o n of the participating companies will be sent to each Medical Director in the near future. This will be the first step in the routine assembling of necessary basic information for this study regarding the employee population involved and the control measures presently in force or as adopted.

REPORT OF CONSULTANT - PROFESSOR PHILIP DRINKER
Professor Drinker said he had no formal report to make but wished to remind the members that he is serving as consultant on industrial-hygiene problems and as such he will be glad to attempt to answer reasonable questions received from members and others cooperating in the work of the API Medical Advisory Committee. He expects to attend both the annual and mid-year meetings of the conmittee and to keep committee members posted on MAC values current or suggested by the various agencies concerned. He or his colleagues will be available for a limited number of brief plant visits and inspections upon payment of travel expense by interested companies;, provided, of course, that any prolonged studies or extended plant inspections would have to be especially arranged to cover costs of services as well as travel expense.

Dr. Thomas J. Kelly of the API Medical Advisory Committee, directed a letter dated March S , 1951 to Mr. E. A. Snyder, Chairman
of the Committee on Agriculture, calling the latter's attention to the following motion which was made November 10, 1950 during the session held by the medical group:
"That a recommendation be made to the appropriate com mittee of the API to determine which petroleum products, and in what amounts, may exist as spray residue in food products."
In the thought that the Medical Advisory Committee may not have been familar with previous excellent work along this line, Mr. Snyder replied to Dr. Kelly under date of March 16, 1951> citing facts presented by five cooperating companies before the Food and Drug Administration during the latter part of 1950. A copy of that complete report is attached.*
We have recently learned that Dr. A. E. Griffiths of Socony-Vacuum; Charles Daniels, Entomologist with the Shell Chemical Corporationj and Leo Gardner, Vice President of California SprayChemical Corporation made up a Technical Advisory Committee to assist in the preparation of facts which were included in the re port presented to the Food and Drug Administration.
The actual presentation before the Administrator was made by Dr. Willard Machle, who is associate clinical professor of preventive medicine at the New York College of Medicine. Dr. Machle had as his counsel, Mr. John D. Conner, Washington, D. C. attorney. The five companies cooperating in presentation of this report did so as the result of an invitation to all interested oil companies issued by the National Agricultural Chemical Association Dr. Machle and John Conner also represented the chemical group in similar presentations to the Food and Drug Administration.
(Dr. Griffiths served as Chairman of the Technical Ad visory Committee. He is located at the company's re search laboratories in Brooklyn and during a recent telephone conversation, told us that he had no knowledge of any recent Interest on the part of the Medical Advisory Committee concerning this subject, and added that he would be very happy to cooperate in presentation of any available facts.)

Doctor Clinton reported on two matters as follows:
1. The publication of reviews which have been accepted by the Medical Advisory Committee prior to the Six teenth Meeting.
2. The two new reviews recently submitted by Professor Drinker on:
a) Oil Mists b] Lead Compounds in Cutting Oils
He recommended:
1. That the 32 accepted but unpublished reviews should be reproduced by varitype-planograph method in limited number of copies to supply members* require ments and public sale.
2. That the two new reviews should be mimeographed for distribution only to Medical Advisory Committee members and associates for Information and comment (see Appondloes A and B).

D. Toxicological Reviews
The services of Professor Philip Drinker have been retained for 195^-1955 and are recommended for 1955-1956. He serves as consultant on industrial hygiene problems and as editor in the preparation of toxicological reviews.
3. Subcommittee Studies
A. Atmospheric Pollutants
The Subcommittee on Atmospheric Pollutants has met and considered a proposed draft of report to Medical and Health Committee of the Board. While the subcommittee endorses in principle the expressed need for liaison between the MAC and other groups in the API dealing with the subject of Atmos pheric Pollutants, we hesitate to endorse the proposal in its present form at this time.
We, therefore, submit for consideration of the MAC as a
whole the following statement amending our report of July 8,
195^ and the proposed draft of the report to the Medical and Health Committee of the API Board to read as follows:
There is a growing tendency to attribute various health problems in communities to atmospheric pollutants including emissions from oil Industry operations and those produced by combustion of petroleum products.
This tendency, to date, is based partly on scientific fact and partly on surmise and conclusion. The subcommittee recognizes a definite limit to the oil industry's respon sibility in investigating community health ^problems but also recognizes the need for improved communications on the health aspects of Atmospheric Pollution within the oil industry.
It is recommended therefore that:
(1) API should be authorized to utilize suitable technical services to:
a) Collect and abstract existing information on the medical and health aspects of atmos pheric pollutants attributable to the petro leum industry.
b) Survey and evaluate medical research now in progress or planned on such pollutants.
c) Study and make recommendations on the need for additional medical research on such pollutants.
(2) The MAC budget for 1955-1956 should include the
sum of $15,000 to cover necessary expenses inci
dental to this work.
