SUBJECT: I-EDICAL SURVEILLANCE OF WORKERS EXPOSED TO VINYL CHLORIDE (April 17, 1974)
All Workers who are exposed to vinyl chloride on a daily basis or who have exposure similar to those exposed daily should be evaluated medically. The procedure to be followed is: .
1. A physical examination should be done on all workers at least once a year. If a worker has been exposed to vinyl chloride for ten years or longer, examinations should be done every six months. The examination should include evaluation of the liver and spleen for possible enlargement.
2. All workers should have certain liver function testing, including total bilirubin, alkaline phosphatase, serum glutamic pyruvic transaminase (SGPT), serum glutamic oxalacetic transaminase (SGOT) and gamma glutamyl transpeptidiase (GGTP). If possible, the liver screening should include these tests; if they are not available, substitute a commonly used test of the laboratory where the analyses are to be run.
If any of these tests should be abnormal, they will need to be repeated.
A questionnaire should be completed on each worker. The questionnaire can be administered by a nurse or the doctor, or it can be completed by the employee himself.
3. Copies of all test scores, as well as the completed questionnaires, should be mailed to St. Louis Medical Department. An evaluation of the testing will be re turned to the plant. Questionnaire enclosed.

QUESTIONNAIRE FOR VINYL CHLORIDE HEALTH SURVEY
The tests which are going to be run on your blood will be used to evaluate liver functions. These tests can be' influenced by past diseases, medications, diet, drinking habits, and work exposure and it is, therefore, important that you answer the'following few questions carefully so that your test can be interpreted:
1. Have you ever had hepatitis or have you ever been 'jaundiced?
If so, when?
2. Have you been hospitalized in the last ten years?
When?

4. Have you had a blood transfusion? When?
5. Have you been exposed to carbon tetrachloride or other chlorinated solvents?
6. Have you taken any prescription drugs or other medication in the last three months?
Name them and include how long you have taken them.
