L/MSA026223
Scope:
Replace a block valve with a cylinder operated block valve using existing (AN) controls. (Note: Revisions to tracing and insulation involving approximately 5 ft. of pipe were omitted from the engineering estimate.)
^Execution: 1. Install nev valve, pneumatic controls and insulate - completed and closed 2/9/84.
2a. Bleed lines were not piped and new valve was not functional. Insulation removed, valve sent to valve shop for correction, bleed lines sketched and fabricated.
b. Bleed lines fabricated opposite-hand; refabricated.
*3. Valve and bleed lines reinstalled and insulated.
4. Drain lines leaked. Remove insulation, fix leaks and reinsulate.
5. No steam flow in tracing lines. Remove insulation, repair crimp and reinsulate.
6. Part of the loading hose and one flange joint were insulated by mistake. Remove and reflnish insulation.
*(Steam tracing was reported to have been omitted on initial installation, which would have added a "strip insulation, install tracing and relnsulate" step.)
