Robinson SHAC The First Quarter Safety and Housekeeping inspection for AN-5 was conducted by the Inspection Team of George Bostick, Jack Richards, Tommy Holder, Harry Robinson and B. J. Sanders on 1/29/85. The overall condition of the south area was very good. The overall condition of the north area was fair. There were no serious problems, but housekeeping in the north was generally below standard. The following items need attention: SOUTH: SAFETY C. 1. Rubber missing from eye bath TT-123. 2. Air hoses not pinned on west side of CT-9. 3. Temporary hoist used by insulators during S/D on east side of 51T54 looks bad and needs to be removed. HOUSEKEEPING 1. Two unracked ladders; one beneath the fin fans and one near catalyst dumpster. 2. Insulator storage area near 51D4 needs cleaning. 3. Miscellaneous trash in open topped bag marked "for asbestos only". 4. Ladder on ground under 51E28. 5. Fresh air mask cabinet near 51T7-2 used to store miscellaneous oil, locks, tape, etc. 6. Kitchen ceiling needs to be cleaned. I SC 000352 LAM001549 NORTH: SAFETY 1. Several flange spray guards missing In sulfuric acid storage area. 2. Eyebath without rubber cup on second level, east side of reactor structure. 3. Utility hose on reactor, second floor needs inspection. 4. Steam hose connection on southeast side of second floor on reactor structure has no bleeder valve. 5. Cover goggle stripe at No. 2 quench area needs repainting. 6. No. 2 quench area sulfuric acid piping has several leaks, poor support and missing flange guards. 7. Maintenance and contract personnel generally disregard use of ear protection. Some were not using cover goggles where required. HOUSEKEEPING 1. Loose scaffold material and trash on north side of sulfuric acid tanks. 2. Trash on No. 2 quench area. 3. Cabinet in No. 2 quench area used by pipefitters was cluttered. 4. No. 3 quench area was littered with loose trash, several hoses were uncoiled and previous acid leaks have eaten the concrete pad. 5. Slab around 51P3 was oily, slick and unsightly. 6. Several left over pallets on north side of area of 51P3 near street. 7. Work on 51Z2-6 was interrupted for unknown length of time. Drain hose was hooked to machine with loose end in a plastic drum, bungs open. 8. Loose scaffold material and bolts lying near 51Z2-1. 9. Plywood ramp in walkway. OTHER COMMENTS AN-5 is a very large area and a concern was expressed about lack of manpower for outside duty during off hours. Many utility hoses have current color code tape but previous tapes have not been removed. SC 000353 LAM001550 Operators indicated need for training on use of Scott Air Packs.
