Forsthoffer - 0-5 The Third Quarter Safety and Housekeeping Inspection for Dept. 15/16 was held on August 15, 1984, by the inspection team of Steve Moncla, Wayne Crowson, Madeline Hare, and Dave Malfitano. The unit received a. rating of good. The following items were noted: SAFETY *1. Unidentified fire extinguisher under counter near chief's desk. 2. Safety log book is very accessible and perceived as a useful tool by the hourly personnel. *3. Only five safety meetings were recorded since 12/83. Hourly personnel were reluctant to talk about safety meeting topics. 4. Personnel have a good knowledge of safety procedures and assembly points. 5. Unit/craft safety team have had meetings in May, June and August. Minutes indicate a functioning group. 6. Interlock/bypass board was visible and in use. *7. Phone number board above control panel out of date. *8. Need to date RV and safety shower/eye bath weekly inspection sheets in Department 16. 9. Consolidation of safety check list sheets will help the record keeping. *10. Safety signs by chemical tanks need replacing in Department 15. HOUSEKEEPING *1. The project to revise the Control Room/Lab/Kitchen will improve the "cluttered" look. *3. Bucket and wire at 15FV393-1. *4. Area by 15E10-2 asphalt is washed out from concrete. *5. Nuts and bolts at 16D8 skirt. *6. Pipe wrench at 16E10. *7. Algae growth at 16Z1, 15E26 and 15E12. This is a slipping hazard. GENERAL COMMENTS Department 15/16 is in the process of insuring that corrective action has been taken on those items noted in the 1983 annual inspection.
