Hughes -SS-13 Three members from the old committee were absent; two were was working the midnight shift. One member of the new incoming group was absent; he was working the midnight shift. The meeting opened with a welcome to the four new members of the com mittee; James Baker, Lloyd Caldwell, Beverly Dodson, and Casey Yauger. OLD BUSINESS Cooling Tower Fan Hazard When the fan drive motors are shut down, the fan continues to turn due to wind drafts through the tower. Attempting to stop the fans for repairs is hazardous. Action The braking system recommended by maintenance was approved by the PMSB. One brake has been fabricated and will be tested prior to fabrication of the remaining thirty-eight. Holeman Responsibilities A question was raised as to what are the responsibilities of a holeman. Action The guideline prepared by SHAC was approved by the PMSB. The guideline has been issued as a SHAC Alert and will also be added to the new Fire and Entry Permit Procedure. SC 12285 lAM013482 Coveralls Coveralls are not being cleaned thoroughly. Many contain stains that will rub off on a person's skin. Some are being issued with the pockets full of sand. Action contacted the laundry about the problem. LP&S has also increased their monitoring of the returned clothes. Operating Crane Not Barricaded The large operating crane being used on the 16D1 job has been set up in the middle of the street with no barricade and/or flashers. A truck being driven by a member of the HSC almost ran into it during the night. Action Maintenance supervision was notified and fl^sffers were installed. Use of "John" When Contaminated If a person has to use the "John" when working with asbestos, NSI, or other similar contaminant, which restrooms should he use. Action The concern was brought to the attention of maintenance supervision for clarification. There are existing procedures for handling both NSI and asbestos, but the procedures are different. The safety committee person representing the operators stated that a well spelled out procedure for handling NSI is available to the styrene operators and that she would bring a copy to the next hourly safety committee meeting. The insulator craft also has a procedure for handling asbestos. These procedures have been recognized for several years as one of the best in industry. Per sonnel required to handle asbestos will be instructed in the proper pro cedure if they have not already been done so. NEW BUSINESS Filter Caps on Respirators Filter caps on respirators keep "popping" off. Action Louis Decker, the LP&S representative, stated that each respirator is care fully checked prior to issuing. If a defective one gets through, return the mask and obtain another one. 12286 LAM013483 sc 12287 NSI How does NSI affect you? What are the dangers? Laborers are the only ones that seem to have adequate procedures for handling. Action This issue was addressed under "old business". Department 44 Flare Corroded The Department 44 flare is badly corroded at the 7, 8, and 9th level. Action The operating department has informed us that a work order has been written and maintenance on the flare is scheduled during the next shut down. Styrene Drum Rack Corroded The styrene drum rack is badly corroded in places. Action The concern has been communicated to Materials Handling. Styrene Shutdown Scaffolding in area left in "wrecked" condition. Boards with protruding nails left on the ground, other boards left dangling. In general, there appeared to be many unsafe practices throughout the area. Action Texas City management is aware of this problem and specific actions have been taken to correct the situation. CED now has a full time safety super visor at the site as well as the contractor performing the work. We can help by notifying our supervision when unsafe acts are observed. We can also help by setting a good example. Resuscitators There are insufficient resuscitators in the plant, particularly in the shops building and other maintenance work areas. Action Resuscitators are now located in those operating areas where certain hazardous chemicals are produced or stored. They do require trained operators and are of limited value. In consultation with the Medical Department, LP&S will review the need for additional resuscitators and make recommendations to the PMSB. J-A/W073484 Rigs Entering Operating Areas Is there a standard procedure for rigs entering a unit? Which leadman should sign permits issued to rigs? If a rig is setting in the road and has a permit to operate, does a new permit have to be issued if the rig swings into the battery limits of an operating unit?
