Glass PLAINTIFF'S EXHIBIT MOJV-1247 The recent incident involving collapse of the Power Department's waste liquid tank and subsequent investigation raised many questions about our relief valve testing program, particularly as it involves new installations and checking valves vs. specifications. The incident report (J. B. Cook, 12/17/76) recommended, under SHAC guidance, "a comprehensive study to develop a procedure to insure safe and correct installation of pressure/vacuum relief devices on equipment. " SHAC, in reviewing this, feels that such a study could be best handled through our Industrial Engineering group and by this letter is requesting that such a study be initiated. Some background information from a SHAC Action Request made last C summer can be made available to assist in this study. We will work with Jack Carlisle and the engineer assigned to define the scope of this study and to assist in revising our procedure.
