Please have these items corrected within two (2) weeks. For the long term items if a two week time table is not possible, we will agree upon a later completion date. This should be done no later than October 12, 1978 Short Term Items (2 weeks) 1. Housekeeping in general needs improvement. Particular areas needing attention are: V-10 dryer and storage area, dry blend second floorLine III, top of old unit reactors, V-ll dryers, new unit recovery area-second floor. 2. Several oil spills were noted in the following locations: east end of Line III mill near the wall, sidewalk north of P-10 tank farm, V-10 air compressor pad, VCM compressor shed around Corkens, transfer pumps southwest of the sphere. 3. Coupling guards were found removed on blend tank slurry pumps 550, 552, 650 and 653. 4. Hole in grating at boiler house deaerator deck. 5. Sliding handrail at north, second level landing at V-10 is frozen leaving an open section of handrail. 6. Two (2) holes in grating on second level of old V-ll building at the west end of the center concrete wall. 7. Nitrogen bottle at 741 reactor NO bottle rack is not tied off. 8. The north side top section of the drive guard on number 6 Bird is missing. Long Term Items 1. Revise liquid VCM drains on Corken compressor K.O. pots to improve flow to recovery system. 2. Provide permanent shelter for flame controller panels in the dryer area. Lurry L. These items have lower priority than the top ten listed earlier, but should also be corrected in a timely manner. Tank Farm Area 1. Regulated area signs not posted. Old Reactor Module 1. The VC recirculation line from the fresh filter pack relief lines to the top of 401 receiver is inadequately supported with rope. 2. Open junction box at the west end of the fresh VC filter pack. 3. Steam leak at packing of gate valve on charge water preheater supply line. 4. Open junction box approximately 6 feet above floor on east side of chem wash tank, V-ll second floor. 5. e is no VCM canister mask between D-400 and 500 at top of stairwa 6. oses used for condenser drilling (air and water) should comply with the plant hose color standard but should be smaller for ease of operation. 7. AMS or OMS spill at drum storage area west of D-600. New Reactor Module 1. DOP spill at grade from 743 reactor seal. 2. Replace grating between 743 reactor and the concrete table top. Replace and secure condenser platform access ladders to reactor top platform. Several breaker boxes and electrical wire raceways found open in chiller area. 2. Hose storage in dryers and other vinyl areas is a continuing problem. Racks are prpvided in many cases but not used. 3. Covers on 4 and 5 Birds are not bolted down causing water to leak through flanges keeping area around Bird and floor at grade level wet. This can lead to algae growth and cause a slipping hazard. 4. Garden hose is still in service on the Bird deck, V-ll. 5. Chain guards on rotary valves (star valves) above V-ll sifters are damaged or missing and should be repaired or replaced. 6. Handrails should be installed at the northeast corner of V-10 roof to prevent a fall when ice forms on the roof during winter operation. 7. Replace cover over light bulb above #2 activator. Maintenance Shop 1. Six (6) open breaker boxes above welding area. Curry L.
