A scaffold board fire occurred in the pipe racks near Department 15 reactors, and this is not the first time that scaffold boards have been set afire by hot piping. In this particular incident with the west wind, the hot embers were blowing toward a contaminated blow down. Since we have this past history, something should be done before we have a real serious incident. 2. What were the findings and what is the status of poor lighting in cooling tower area east of the Applications Building? 1. Pressure and rules are on the maintenance crafts to wear respirator equipment when possible exposure to benzene. What is this not also applicable to material handling employees? 1. The exposed light in the janitor's closet at the west entrance to the Office Building is still a hazard. Nothing has been ddne about i t. NEW BUSINESS 1. Zeller gave a report to the committe on his findings based upon a request of the SHAC Committee regarding locations in the plant with high potential to cause major injuries which occurred to Robinson and Heer. The report has been submitted to the SHAC Committee and the Hourly Committee is now awaiting their action. 2. The lighting in Department 10 compressor building is extremely poor and something needs to be done without delay. SC 12721 LAM013916 Several valves in Power 2 are both in awkward locations and very difficult to open, requiring in some cases the use of 1(8" pipe wrenches. This same equipment also leaks after the valve is closed and necessary repairs should be made. Several people have requested that a wind sock to show the direction of wind be installed in the styrene plant so that employees would know what direction to move in case of a serious spi11. Currently the shop is being sprayed for mosquitoes shortly after 0800 when it is full of employees. Can arrangements be made where the shop could be fogged prior to the time that employees arrive? Departments 14 and 16 (old 4 and 8 structures), concrete chunks fall out at various times. If someone is in the area at the time, it would cause a major injury. Also, concrete "chunks" are falling from the walls in the pump pits at the salt water pump house. Recovery units in Department 15 (acid tanks) have leaks and are seeping. Bolts on flanges have completely deteriorated.Action to correct the hazard is too slow. High pressure steam is being used in low pressure piping to turbines in the salt water pump house. A leaking steam line in the north pit at Power House 2--should it become worse or rupture with men in the pit, it would create a serious problem. It should be repaired. It has been leaking for too long a time. Discharge line from steam pit and sump pump in north pit of pump house should be extended to the salt water entry. What method is being used to determine condition of steel plates under the asphalt covering on the east side of the salt water pump house. With the number of vehicles in the plant, there is a serious problem in the fact that a vehicle will drive over this area, and it is about 20' to the water level below. What is the hazard currently and eventually to the foot traffic and heavy tool box traffic in this area. SC 12722 LAM013917 3 SC 12723 A new bottom has just been put in 16T18-3, and employees working in the tank state the walls are in bad shape, having had to set the welding machines to as low as possible when welding back a plate they had removed to get repair materials into the tank. Employees who rod out 51E28s wear their same slicker suits day after day on other jobs and sometimes receive irritation from, contamination around the collar and cuffs. Is there some type of throw-away clothing that can be used on this job? 6. Some equipment brought to the job is heavily coated with some type of coating or paint, and welders burning and welding on this equipment are exposed to large amounts of fumes. Can something be done to sandblast off this foreign material before welding and burning? 7. Working on top of the storage tanks in Department 35, handling the fendwall is extremely hazardous. The platform needs to be lowered, in addition, better work area is needed when filling fendwalls since fumes create a serious problem in the present work area. 8. Just outside the door of Department 35F, condensate tank is in bad condition. 9. It appears that a new tower is needed to replace 35D1. It is leaking badly through the insulation and has been patched several times. 1. Drinking water cooler in Warehouse A should be relocated.
