Corwin Hinshaw M.D. Industrial Indemnity Indemnity Rescomb Borden and Clinical History CONTINUED Occupational suffering from congestive heart failure of pulmonary origin at that time Treatment Entiat required oxygen in the usual measures for treatment of congestive heart failure I am | - informed by Doctor Crenshaw Finally on February 13 1960 an operation was performed at Alta Bates Hospital by Doctor Crenshaw which consisted of opening the left cheat removing a portion of lung including some large emphysematous blebs Doctor Crenshaw informa me that the pathologist at Alta Bates Hospital Doctor David Singman made diagnosis of asbestosis on the basis of this material remove, Doctor Creashaw further ' tells me that the lung was found to be quite severely scarred and difficult to expand Following the operation the patient developed a gastric ulcer while still in the hospital and this ulcer apparently ruptured and required emergency abdominal surgery which was carried out on or about March 20 1960 by Doctor McChesney The patient had a well difficult postoperative course but eventually regained his generalsense of believea that be is somewhat improved generally ; being and| Present Symptoms General Complaints - He states that at present he feels comfortable so long as he does not exert himself that he has no fever that his weight is approximately normal and no other sort of distress respiratory Sympt ms- His principal complaint is that shortness of breath and aad he says that he would not be able to climb more than one flight of stairs or walk more than half to one block without becoming severely distressed from shortness of breath _ _ _* s that he would have to stop and breathe before continuing He complains of a sorenss8 in bis chest principally on the side where the operation had been performtehde left side He says that he has ittle or no cough at present but did have a severe cough before he | quit smoking about one and half years ago He did cough very severely in January 1960 He has no current symptoms of congestive heart failure such as ankle swelling anginal pain etc. intestinal Symptoms - He has no ulcer symptoms at present such as abd minal patn Indigestion etc. He also has no difficulty with his bowels urinary Symptoms - No difficulty passing his urine no frequency no pain etc. . Ears Head Eves Nose Throat - No unusual complaints He rarely has respiratory , infections Central Skeletal System Muscular System Sestem - No complaints Nervous System Peripheral Nervous ; Personal Habits 7 He does not smoke He never usi a alcohol to excess He leads a very quiet life He say that he has not been employed since January 1960. Corwin Hinshaw Industrial Indemnity Roscomb Borden M.D. Past Medical History He has never had rheumatic fever diabetes typhoid fever malarla syphilis hypertension asthma hay fever arthritis pneumonia tuberculosis or any other serious disease He had never had any operations prior to the operations mentioned above . * Family History Family disease tendencies - none known His mother died of beart trouble at the age of about 55 years His father died as a result of an accident in a logging camp whena young man He has three brothers and three sisters who are living and well ' THE PRECEDING PORTION OF THIS REPORT WAS DICTATED IN THE PRESENCE OF THE PATIENT AND HIS WIFE ON DECEMBER 22 1960 Physical Examination health His general appearance is that of a person in excellent general Height - 6 ft pounds Weight - 181 pounds usual weight 176 Blood Pressure - Normal 140/70 Pulse - Normal 68 Temperature - Normal 98 Eyes - Pupils are round regular and equal reflexes to external ocular movements normal Oral Cavity- No significant changes seen Lymph Nodes - None are enlarged or diseased Thyroid- Normal light and accommodation normal Breasts - Normal Skin - Normal no cyanosis observed Chest Wall Shape within normal limits Scar of left thoracotomy is well healed Respiratory excursion considerably limited Slight tenderness in the region of the scar Percussion note considerable dullness over the lower half of the left chest laterally and posteriorly Breath sounds are coarse at the left base distant throughout the right lung absent at the left apex and abnormal Heart Size Indeterminate rhythin regular no murmurs or other detected Neck veins are not distendeddistended in the erect posture sounds Abdomen - No enlargement of liver spleen or kidneys no ndegrlubnileiass abnormal masses detected a . . Watson Pathologist namely namely the findings on microscopic examination of lung tissue removed at the tune of Mr. Borden's lung surgery have also reviewed a report of chemical examinations of the tissue carried out by the State of California Department of Public Health a The information supplied from Alta Bates Community Hospital 1s probably not the complete hospital record but it seems to be adequate for the present purposes and it concerns Mr. Borden's hospitalization there in February and March 1960 including a detailed report of the operation performed by Doctor Gerald Crenshaw on February 25 1960. The records confirm the history as recited in my previous report and the numerous ray reports confirm the fact that this man had very extensive pulm naryinjury with extensive cystle changes for which the operation of February 25 was performed At the time of operation Doctor Crenshaw observed the extensive disease which rays have demonstrated He carried out segmental the parietal resection of a portion of the left upper lobe and removed a portion of pleura The findings on microscopic examination of the removed lung tissue have been reviewed and the material has been submitted also to Doctor George A. Borden Dear Mr. Lambor This is a resume on Mr. Borden as you requested Mr. Borden came to me having been cared previously by Dr. Howard Huenergardt because of progressive dyspnea He was seen in this office from January 23 1960 complaining of chest pains cough with and dyspnea He said that he had this trouble for sputum been told to many years and had see a specialist by Dr. Huenergardt He didn't improve so was referred to us He gave a history of having worked in the Kiandiussetrry for a period of twenty eight years He had been referredatsobetshteos Hospital by the Insulators and Asbestos Industry of Northern Calif- ornia Local Union 16 Health and Welfare Plan at 200 Guerrero Son Francisco California on March 5 1955 for Street . and Xrays of the chest pulmonary function studies : it liters impairment The pulmonary function studies done in 1959 showed that he had capacity of 4.1 liters and in 1959 was 3.9 3.1 liters in 1958 and 2.9 a lung Timed capacity was liters in 1959. Their percent was 76 to 95 which indicates a moderate lung impairment 75 and below indicates impairment At that time he had severe lung severe lung His Xray was read by Dr. Richard Holmes marked as generalgeinez raleizded emphysema and there appeared to be slightly more fibrotic chonges in the lower lung fields than on previous examinations of October 2 1957. The - ed impression was mark- generalized emphysema with probably slow progressive fibrosis The patient was hospitalized further by me at Alta Bates Hospital on preparation for examination because at that time he presented with atrophy of both apices and marked large bullous bullous cardiac failure There pulmonary fibrosis in both lower lobes and swelling evidence of was definite ankle and edema also definite cor pulmonale and congestive failure He positive mittent pressure therapy until his cardiac was placed inter- . improXrvaeysd~ condition known Surgery was performed in order to obtain a piece of lung because the at that time were characteristic of asbestosis As etiological features of for advanced well the formation & asbestosis show bullous cyst formation 7 at tunes and a mottling the lower lung fields irregularity irregularity mediastinum to man's described as a shaggy border to the heart and in marry ~ : coses as in (hy 5 this mediastinum iyi : atl cet et ae z 5oa ran > , 1 detnente tei ntraanirtetmmnetse a Re Rescomb E. Borden case there is evidence of pleural thickening Pulmonary function studies show the characteristic findings again of pneumoconiosis and asbestosis which shows that the patient cannot really take a deep breath and there is an extraordinary diminution of inspiratory capacity compared with the normal person There is often a normal maximum breathing capacity but a marked diminution in inspiratory capacity and usually no respiratory flow resistance and characteristically a normal or relative good maximum breathing capacity is present as was in his case He showed hyperventilation on excursion The patient was admitted under degeneratiluvneg disease until a more accurate diagnosis could be made His electrocardiograms at that time showed a slight shift to the right probably due position The depression of the S segments in the right leads in both electrocardiograms were due to right heart strain or cor pulmonale They were read by Dr. K.W. Berson of Alta Bates Hospital He was taken to surgery on February 25 1960 at which time a large bullous atrophy of the upper lobe was determined and resected in the classical manner The lower portions of the lung were found to be markedly fibrotic gritty and grossly involved with some irritating chemical Based upon the patient's history it was assumed that this was asbestos Lung biopsies were taken and sent to the State Laboratories which confirmed that this man had extensive pulmonary fibrosis associated with a large number of asbestos particles The patient's post operative course was extremely stormy Even at this time the remaining portioonf the lung had not filled the left chest cavity because of marked constriction due to the fibrosis in his lung He developed on electro- lyte imbalance necessitating consultation He also had a rupture of the cardia of his stomach which again necessitated consultation and surgery from a general surgeon This rupture failed to show on regulor G.I. series and caused a great deal of controversy in the hospital until the actual exploration when a rupture of a small area was found to be present below the diaphragm in the cardia of his stomach This was repaired and the patient made an uneventful recovery - He has been under care in my office for extreme pulmonary crippling due to inhalation of asbestos particles He has gained weight looks well but on the least exertion he becomes markedly dyspneic and his activity is markedly impaired It is my considered medical opinion that he never will be able to return to any gainful occupation The ultimate prognosis is poor in his case because of the extreme crippling as the result of his extensive fibrosis Conclusion This is a case of industrial origin due solely to the inhalation of irritating asbestos particles over many years associated with his work This has resulted in atrophy which occurs in asbestosis and in pulmonary fibrosis-7.
