THOMANN, M.D., MEDICAL DIRECTOR DEER PARK MANUFACTURING COMPLEX SUBJECT: PULMONARY REGISTRY REVIEW EMPLOYEE NUMBER The Pulmonary Registry form indicates that worked for Shell from 1946 to 1979. Further information supplied to me includes a note from Dr. S. R. Cowles dated April 4, 1988 which included the statement that Dr. Cowles had been informed by the family that there was in case X-ray findings indica tive of asbestosis. According to a memo from Katherine Moore to Dr. Cowles dated February 24, 1988, last examination at Shell was a silica surveillance exam in 1978. His chest X-rays do show evidence of diaphragmatic and probable chest wall calcification. In addition to his work at Shell where he was employed as a painter, there is a history indicated of shipyard work during World War II. It is now approximately 14 years since has left Shell employment. At this point in time, there would not appear to be any relevant recording or reporting requirements, either from an OSHA point of view or from Shell internal standards regarding recording of occupational illnesses. I do not believe that there is any aspect of this case that would require any action at this time.
