Comments on Statistics - M. A. Riddle There were no recordable, injuries in September and the total injuries year to date are the lowest ever for the Texas City plant. Contractor Safety There were four injuries in the maintenance contractors supple mental force and only one minor injury in the capital projects contractor work force in September. Crane Incidents Three recent major crane incidents in our plant were discussed. (1) Dept. 51 - 8/2/84. Cherry picker overturned while attempting to move a motor from pump location at CT9. See incident investigation No. C84-5. (2) Dept. 44 - 10/13/84. Tower crane incident. Incident investi gation is underway. (3) Dept. 50 - 10/16/84. Crane boom struck ladder and platform on '`heavy ends" 50D203 column. III. REPORT OF THE SAFETY ADVISORY COMMITTEE (SAC) - 6. T. Ryan A. Rattling of Exchangers Plant policy published by SHAC. ( B. Backflow Preventers All manufacturing units were surveyed to see whether or not backflow preventers should be installed on plant air. The only units where it was indicated there is a need for backflow preventers were Methanol, PA and Oxo-Esters. The survey indicated there was no need for other units to comply. IV. REPORT OF THE PLANT SAFETY AND HOUSEKEEPING ACTION COMMITTEE (SHAC) E. P. Bailey A. EPP Drill - Steve Moncla Recommended November 15 and will involve many of the plant's safety organizations. The scenario will be a spill at the cryogenic ammonia tank. We will involve one outside group at Danforth. It will be necessary to notify the media. B. Annual Safety and Housekeeping Inspection Conducted September 19-21, 1984. Feedback session in cafeteria was well accepted with good positive remarks from the inspection team. A critique was held and documented for next year's committee. C. Eyebath Freeze Protection - Thurman Payne The new plant procedure is being circulated for review. SC 002421 LAM003628 PMSB Meeting Minutes - October 16, 1984 Page 4 D. Contractor Safety The AN-5 shutdown critique was used to improve the overall approach to the Methanol-Acetic Acid shutdown. An attempt is being made to improve the five-minute safety meetings at the start of each shift. Contractor Videotape for Visitors, Truck Drivers, etc.: The script is in progress. E. Hourly Safety Committee There was one item of old business which involves traffic signs in the plant. There are a few Stop signs in the plant that have the wording "Stop for Pedestrians" on the Stop sign. It was recommended that we should use only a standard Stop sign where we want vehicles to stop and a Yield sign where we want vehicles to yield. V. REVIEW OF INCIDENTS Monsanto Incidents: Date 8/8/84 Incident No. 84-7 (issued) Contractor Incidents: Description Pipefitter received laceration to cheek (5 stitches) while loosening bolts on a flange with impact wrench. (Richards) Date Incident No. Description 8/23/84 C84-06 (issued) Explosion and fire in south breaker room of AN-5 when 4160 volt motor starter faulted during initial startup of new motor. (Noel) VI. MISCELLANEOUS - M. A. Riddle A. STOP Program 1. Hourly Unit I - 90% complete. Unit II - 75% complete. Unit III planned between the "Methanol and Styrene shutdowns". 2. Non-Exempt The program is underway. Units I and II are complete and Unit III will be conducted during the weeks of October 29, November 5, and November 12. 002422 LAM003629 PMSB Meeting Minutes - October 16, 1984 Page 5 3. Exempt A program for all the exempt people that have not had the STOP training is planned to begin in November. 4. Orientation of New Employees Gene expressed need for all new employees to be given an indepth safety orientation within their first few days of employment. LP&S is currently giving orientation. B. Quarterly Safety and Housekeeping Inspection SHAC was asked to consider the two following concerns: 1. The department would like an immediate feedback of any type of problem areas at the conclusion of the inspection. In one case, there was very little said by the inspection team and then a long list of items came out in the report. 2. The need for continuity was discussed and it was felt that at least one person of the team should be rotated forward. This would aid in the evaluation of how a department had improved and responded to the previous inspections report. VII. PLANT MANAGER 'S COMMENTS - G. L. Tromblee Gene indicated that we have an excellent momentum going in the area of safety and that we need to keep this going through the Methanol/Acetic and upcoming Styrene shutdowns. /dgb M. A.
