SECTION I - INTRODUCTION
Site Entry and Inspection Objectives
I, EPA Region 5 Lead Inspector, Dean Maraldo, arrived at the Forest County Potawatomi Community ("FCPC") Utilities office near the Crandon Recirculating Sand Filter/Stone Lake Wastewater Treatment Plant ("Stone Lake WWTP"), located at Hwy 8 East, Crandon, WI 54520, at 12:22 PM (CT) on 09/06/2023 for an announced inspection. I, along with EPA Region 5 inspector Ray Cullen, presented our inspector credentials to Nate Guldan and informed them that this was a EPA Region 5 inspection to determine compliance as authorized by Clean Water Act (CWA) Section 308 and implementing regulations. This report is based on information supplied by representatives of the Forest County Potawatomi Community, owners and operators of the Stone Lake WWTP and Carter Wastewater Treatment Facility ("Carter WWTF"), direct observations made by EPA Region 5 inspectors, records and reports maintained by the permittee and other information including: photographs taken by EPA Region 5 inspectors, physical evidence collected by the EPA Region 5 inspectors, verbal or written statements made by information supplied by Stone Lake WWTP and Carter WWTF representatives during or subsequent to the on-site inspections, and materials, processes, data, photographs, or documents shown, demonstrated, or submitted to the EPA Region 5 inspectors by Stone Lake WWTP and Carter WWTF representatives during or subsequent to the on-site inspections. In addition, information gathered prior to or subsequent to the inspections from a review of EPA and/or public records may be included in this report.

I began by reviewing the facility description provided in the current permits (excerpts below) for Stone Lake WWTP and Carter WWTF, and the Facility representatives confirmed the information as described below. Changes to the permit descriptions were made by Bruce Johnson (Utility Manager) and indicated in bold below.
Permit Fact Sheet Facility Description for the Carter WWTF:
The applicant recently constructed a new wastewater treatment system as described below to replace its existing lagoon facility.
The system is designed to treat 0.06 mgd of domestic wastewater. The discharge uses the same outfall as the replaced lagoon facility and is continuous to a wetland from Outfall 001 on the Reservation. The facility was placed into operation in June 2019.

The FCPC Carter Wastewater Treatment Facility (WWTF) provides wastewater treatment services to the local community and the Casino. Lift Station #1 transfers flows from the sanitary sewer system to the WWTF via 8-inch sanitary force main to the WWTF Headworks where influent flow is measured with a magnetic flow meter.
The Head works consists of a combination fine screen and grit classifier unit with a capacity of 0.4 mgd (error, should be 0.7 million gallons per day "MGD"). The screenings and grit are discharged into separate dumpsters for disposal. An influent composite sampler samples the influent wastewater immediately after screening.
Septage from the Carter area is hauled to the WWTF site. A Septage Receiving Station with a magnetic flow meter to record septage volume and to facilitate metering of flows into the treatment process.
After screening and grit removal, the wastewater flows by gravity through an 8-inch pipe to the aeration splitter box. Stop gates allow influent wastewater to be channeled into any of the three (3) aeration basins.
At startup, only one (1) of the aeration basins will be in service while at design capacity, two (2) aeration basins will be used. The third aeration basin can be used in the event the other basins need to be serviced.
The aeration basins are designed with anoxic and aerobic zones. Three (3) pH adjustment chemical feed skids and one (1) Phosphorus removal chemical feed skid is provided.
Following aeration, the wastewater flows into one (1) of two (2) final clarifiers (only using one clarifier right now, one on standby). Only one (1) clarifier is required to be on-line at any given time; the second unit is provided as a backup for maintenance purposes. Following clarification, effluent flows to an Ultraviolet (UV) disinfection unit. The UV disinfection system has a design average flow of 275 gpm and a peak flow of 350 gpm.
After UV disinfection, effluent is discharged to the current outfall by gravity incorporating a series of manholes. Spill plates are provided in each of the manholes to increase Dissolved Oxygen (D.O.) levels in the effluent prior to discharge.
Jennifer Spencer added that there are two lift stations in Carter WWTF collection system.
Permit Fact Sheet Facility Description for the Stone Lake WWTP:
The application and plans indicate that the permittee owns and operates a 0.07 mgd wastewater treatment plant using a recirculating sand filter.
Wastewater is collected via two lift stations, twelve effluent pumps, and several thousand feet of gravity sewer and force main.
The Stone Lake Wastewater Facility Modifications provided for Septic Receiving, Phosphorus removal and UV disinfection. The project also redirected influent wastewater to be screened prior to further treatment with the existing Recirculating Sand Filter.
To accomplish this, the project included a new metal building with wastewater treatment equipment and exterior site improvements. In addition, the project
included converting the existing Controls Building to storage and adding controls to tie the Main Lift Station Controls to the Septage Receiving. The Recirculating Sand Filter stayed in

place as-is. The discharge is continuous to a wetland from outfall 001 on the Reservation. The sludge from this facility is transported to the new Carter Wastewater Treatment
Facility for dewatering with the Carter sludge. Final disposal is to a municipal solid waste landfill.

Stone Lake WWTP and Carter WWTF Inspection Date: 09/06/2023 SECTION V - CLOSING CONFERENCE AND FOLLOW UP Closing Conference I began the closing conference with FCPC personnel at 03:30 PM (CT) on 09/06/2023. During the closing conference, I discussed the preliminary observations and Areas of Concern identified during the inspection and summarized above. Preliminary Areas of Concern have not yet been evaluated for a formal compliance determination. I reviewed the timing for completing the inspection report and went over the documents received during the inspection (See Appendix 2 - Document Log). I received copies of records requested prior to the inspection. I asked if there were any questions before concluding the inspection. FCPC representatives asked for an appropriate contact at EPA to discuss permit issues. I referred them to the EPA Region 5 Permits Program and provided contact information. With no further questions, we concluded the inspection at 03:45 PM (CT). Follow Up No follow up was required after exiting the facility on 09/06/2023. Sanitary Sewer Overflow (SSO) Procedure
In the event of any SSO, please follow the below steps in compliance with the NPDES Discharge Permit:
1. Notify immediate supervisor of event 2. Supervisor will notify the EPA in accordance with the NPDES permit:
a. Report any noncompliance which may endanger health or the environment, within 24 hours from the time the permittee becomes aware of the circumstances by calling 312886-6106. A written submission shall also be provided within 5 days of the time the permittee becomes aware of the circumstances to address mentioned in the permit.
b. Written submission shall contain a description of the noncompliance and its cause; the period of noncompliance, including exact dates and times; if the noncompliance has not been corrected; the anticipated time it is expected to continue; and steps taken or planned to reduce, eliminate, and precent recurrence of the noncompliance.
