Kaldenberg Bubba Bean Paul Siems Steve Moncla The Fourth Quarter Safety and Housekeeping Inspection for Department 513 was conducted on 1/23/84 by the Inspection Team of Paul Siems, Bubba Bean, and Bob Hammann. Overall the department was considered outstanding. The following are comments from the inspection. Please respond to these recommendations in writing to George Pappas, with a carbon copy to Steve Moncla by 2/21/84. General Comments It was noted by the Inspection Team that the operators and supervisors take pride in their department. They are setting a very good example for the rest of the plant to follow. We would recommend that the unit lab be inspected by the Technical Center ( Safety Committee. Although the vessel inspection program is behind for the unit, plans are under way to get these vessel inspections up-to-date in the near future. We would recommend that this proposed schedule be followed closely to assure compliance. We also recommend a loss prevention review of the relief valve testing program. Frequency for testing some relief valves could be extended. Personal Knowlege of Safety 1. The critical alarm/interlock bypass board has been located in the control room. Written procedure should be developed detailing authority required for instituting any bypass. 2. Monthly safety meetings were held in 1983 but attendance was poor. Need to assure that the majority of operating personnel are covered by these safety meetings. In addition we recommend that other sources be used for conducting safety meetings instead of putting the burden on one person. 3. The procedure of checking the Malone trucks on a weekly basis is to be commended. Personal Safety Equipment , No deficiencies noted. SC 000614 LAM001812 Bill Wooley Page 2.
