Sather Date : May 29, 1984 Subject : SAFETY COMMITTEE MINUTES The ESAR subcommittee met at 10:00 a.m. Answers are due to VAB by 5/25/84. The full committee met and followed the attached agenda. Injunes/Exposures - AHS and PEM shared a concern about the potential for complacency in the plant, particularly m light of the recently achieved record in the medical case area. CRS shared a concern about the current trend of VCM overexposures. JF said "I couldn't be prouder" about the excellent leadership results regarding the one year without an OSHA recordable injur>. Complacency is also a concern of his. He polled the group about their feelings m this area. Most of the group shared the concern and will cover it m safety meetings. JF also was concerned about VCM trends, particulaily in the area of sample handling errors. Safety and lab are to analyze this to see if the system can be improved. Subcommittee * s Progress - Interlock and Alarm Setpoints - Phase I of this project will be implemented in the vinyl reactor area. Current plans call for the checkout plans to be m place and the checks will be done while a reactor is down for rupture disc changes or other similar one day type outages. - Blinding - as noted in the agenda. - Audits - Due to the dafficulty m being able to meet, JF and AHS to discuss direction. Departmental Safety Reviews - PJK asked what other departments were doing with Job Safety Analyses (JSA's) He said that he felt that progress year-to-date was slow and that come year end, we would be inundated with rev:ew requirements. - CRS said that his department's approach to this was to write standard operating procedures as opposed to JSA's. - PEM said that there was no program to do JSA's m Vinyl as there was a heavy load of startup and other related activities m the mill at present. Plans call for implementation of review and training m July with startup m September. Department Heads are to provide training input to AHS re garding how best to do the training for their personnel. - Dump Sweep Preparation Procedure - As a result of a first aid type injury to a Maintenance mechanic. Vinyl and Maintenance have developed a procedure for proper preparation of this equipment. Some discussion occurred regarding where this procedure is to be kept so we have a ready reference m the operating areas to properly prepare equipment for work. JF suggested that this type of information is best kept by the operating departments in the form of a depart mental safety procedure book. - Safety Milestone Planning - CRM suggested that the plant begin planning for five years without a disabling injury. The group felt that the steak dinner was a success and should be done again. They also felt that the awards should be changed to a jacket or similar type item as opposed to a gift catalog. A. H. The Safety Committee met after the FSARs were reviewed. Injury and Exposure Trends Injury performance during April and May to date is four first aids. On April 24, 1984, the plant achieved one year without an OSHA recordable injury. Year-to-date injury distribution by department is: Maintenance - 8, Compound - 1, Lab - 1, Office - 1. The following is a brief summary of each injury repoit, - A mechanic was welding in the welding shop. While in a fixed body position, he reached out and felt a pull m his upper right back. Tins was a minor injury that was not investigated further. - A mechanic was working on the new reactor module Sweco boot during a shutdown. At the same time, revisions were being made to the chem wash storage tank drain. During this period, the chem wash was stored in 744 reactor. Some caustic got out of the reactor into the dump line. The mechanic suffered a minor caustic burn to one of his fingers. Prior to work on the Sweco in the future, the dump line will be flushed with water and isolated from the Sweco. A team investigated this incident and will provide rec ommendations regarding Sweco preparation. - A process engineer was pumping pond resin at #1 pond. Whilp adjusting the height of the pump, the winch handle slipped out of his hand. As the pump went down, the handle hit his wrist several times. The pump winch was changed to a worn gear drive versus the ratchet system that caused the injury. - A mechanic contacted an exposed steam tracing line while repairing a union at the old module PVA tank causing a minor burn. Tins incident was not investigated further. In the lead area, two overexposures were noted on personnel who wore respiratory protection. Subcommittee Progress Interlocks and Alarm Setpoings - CRS will update this in the meeting. Blinding - A total of fifteen checksheets have been submitted for review. These cover seven of ten reactors, the Emission Recovery System separator tanks, the new deaerator tank, vinyl area VCM receivers, We lex intensive mixers, P-1 relief tank, and Line I blenders. Revieu efforts have been concentrated m the vinyl area first with the Emission Recover) separators and reactors. Audits - The committee has made no progress since last report. Tool Inspection - Dan Miller will be added to the committee. He will work with AHS, CLD, and MHW to develop the procedures. The original committee will be responsible for review and execution.
