Carl Richardson and Ruby Mosley of the outgoing committee also attended, in order to help in the transition. The guest speaker was Arnold Glass, LP&S superintendent. He reviewed the purpose, function, and membership of the Texas City plant safety organization and com mittees. Special emphasis was placed on the hourly safety committee--the history and the responsibility of its members. In addition, he answered questions related to a recent plant incident, rules requiring the wearing of hard hats, and the status of the STOP program. Dennis Kos reviewed the revised Procedure Bulletin on "Use of Potable Water." The HSC charter and the qualifications for concerns which can be addressed by the HSC were outlined. The first order of business was the selection of a new HSC chairman. Carl Richardson (outgoing chairman) accepted nominations and held the election in keeping with Procedure Bulletin P-0100. Gene Crain was elected chairman by unanimous vote. Unfinished Business Machine Shop Door Access (James Niddifer) o This concern relates to a concern which addressed earlier this year. Truck entry beyond the machine shop doorway located on the north side of the main shops building is pau:tially blocked on the east side by a lunch box cabinet and vending machine. This makes truck delivery of equipment inside the machine shop very difficult, and potentially unsafe. o The machine shop doorway and the area inside the building was surveyed and measured. The vending machine and cabinet extended about 6" beyond breaker boxes, etc. Relocation of these breakers would be very costly. Clayton Pucket has agreed to restrict truck entry from reaching the obstacles on the east side. The restricted floor area will be highlighted with painted borders and instructions. Trucks will not be allowed past this point without super vision approval. Delivered equipment can then be relocated with the shop overhead crane. sc 002036 lA1\/103241 Hourly Safety Committee -2- August 20, 1982 New Business New HSC Area Representation o There is some uncertainty on which groups each HSC member represents, especially following the recent reorganization. Could you reissue the representatives list? o Mark Riddle has informed me that the new HSC and unit/craft team represen tation lists will be issued in August. To provide immediate help, the attached table covers the groups represented by each HSC member. Dirty Intersection Mirrors (Pervis Cotton) o A number of the curved mirrors located at intersections throughout the plant are so dirty that they are of no use. Who is responsible for keeping them clean on a regular interval? o This comes under the responsibility of "yards and grounds" maintenance. Joe Hogan has agreed to have them deemed by August 3 and to set up a cleaning as it is needed. Inexperienced Operator Staffing (George Williamson) o With the high turnover of operators in the plant some units have frequently staffed shifts with nearly all apprentices or inexperienced operators. Even though these operators receive one or two months of training before filling a shift position, this practice can result in a delayed or ineffective re sponse to emergency conditions. o We also share this concern. In an attempt to offset this increased inex perience level, we have implemented stepped up apprentice training, operator refresher training and chief operator/leadman training programs. However, because this concern relates more to staffing than safety, specific staffing concerns should be addressed to unit supervision. Department 33 Safety Shower (George Williamson) o In Department 33, the area around 33 P2S is a considerable distance from a safety shower. Addition of a new shower was suggested to supervision many times in the last two years. We were told the new shower was in the works. o This item was discussed with Ken Bretz and George Chesnut in Department 33. There is no present project to install a new safety shower in this area. This suggestion was not recorded in the safety logbook and was not mentioned in unit/craft minutes of recent years. This concern highlights the reason it is important to make safety concerns visible. Safety concerns must be addressed at the unit level before pro gressing to the HSC. For a concern which does not require immediate action the proper stepwise procedure is 1) safety suggestion book, 2) unit/craft committee, and 3) hourly safety committee. This order is necessary to promote the solution of problems at the unit level, where the best answer can be found in the shortest possible timeframe. LAM003242 SC 002037 Hourly Safety Committee -3- August 20, 1982 Invalid AN WOB Entry Permit (Walt Middleton) o During the week of July 5, work was progressing on the #2 WOB in AN-5, and an entry permit was issued for the carpenters to modify internal scaffolds. Just before entering, boilermaker Bolen yelled at the carpenters, telling them not to go inside because the tubes were being hydrotested. The carpenters should not have been told to enter and given a permit. This was reported to Felix Dezengotia in maintenance. o Mark Devore investigated this incident. During this time frame, weld repairs were made to the #3 WOB economizers. Entry permits were issued covering 24 hours for each craft. During the hydrotesting by boilermakers, the other craft entry permits were to be pulled. This investigation could not be thorough due to a lengthy delay in reporting of the incident to supervision. Incidents should be reported immediately to manufacturing and maintenance supervision so that an immediate investigation can be conducted. Mark Devore indicated that action will be taken to prevent recurrence. Main Shop Storage of Gas Cylinders (James Niddifer) o On the south side of the shops building gas cylinders (acetylene, oxygen, etc.) are stored under the pipe rack. Is this an acceptable practice? o LPsS department personnel have surveyed.this area. The storage of these gas cylinders under the overhead pipe rack is acceptable. However, it was found that the wall which segregates acetylene and oxygen cylinders needs to be extended. This will be handled by Ivey Partain. The next HSC meeting will be held Tuesday, August 31, 1982, in the small conference room of the Personnel Building. Attachment D. D.
