Ouly, Finland August 7" 2023 Polyvinylidene fluoride (PVDF) hernia meshes like Dynamesh IPST is used for repair as well as prevent parastomal hernias (PSH). This specially shaped mesh can prevent a significant number of PSHs compared to controls without the mesh, (1-4), leading to better quality of life, less stoma related morbidity and consequential savings in health care costs. Previously widely studied flat mesh (keyhole technique) does not in PSH prevention (5-7). Yet, the European Hernia Society recommends to use a prophylactic mesh when a permanent colostomy is constructed. Dynamesh IPST is unique in design and so far there arent comparable products. The architecture of the mesh has led to significant improvement in parastomal hernia prevention. The randomized controlled trial (Chimney Trial) recruited 143 patients in Finland and Sweden (9). The 12-month results of the study will be published shortly and will show the benefits of the mesh without mesh related complications. The withdrawal of PVDF as a material of hernia meshes and denial to use Dynamesh IPST would lead to significant drawback in parastomal hernia prevention. About 50% patients with permanent colostomy without a prophylactic mesh or with flat mesh (keyhole technique) will have a parastomal hernia (1-4). The results of parastomal hernia repair are poor, leading to recurrencies, re-operations and complications in a number of patients (10). ao vTea Elisa Mkrdinen, M.D. Ph.D. Oulu University hospital, Finland Tero'Rautio, Professor, Chief of Surgery `Oulu University hospital, Finland 1.
