At the time of this meeting it was 68 days. He commented on the fact that the trend was in the right direction. Phil also reported that the MCIC Plant Manager's Task Force on Safety had reviewed the first quarter '77 figures showing major injuries (lost-time) in MCIC were higher than last year. One of the four major injuries was at Texas City. The Plant Managers compared notes on steps being taken to improve overall safety performance. Phil pointed out that our "Take the Extra Step" safety contact program was aimed at improved safety performance and to keep our record going. To make it work, however, it takes interested involvement of supervisors at every level. Phil said that his Friday visits to the plant convinced him that the personal contacts about safety are important and are fundamental to the success of the program. SAFETY ADVISORY COMMITTEE (SAC) -- G. W. Bostick The following items were reviewed by SAC at their last meeting and a brief status is outlined. 1. Common Carrier Wrecks R. L. Stuart and Material Handling group reviewed present response, the P-bulletin 906 and status of corporate efforts on the problem. In-Plant Railroad Crossing Task Force Of the 15 concerns developed by the task force all had been resolved except five. Bill Riddick reviewed the status of these items and was instructed to obtain engineering estimates of the costs so they could be reviewed at the next SAC meeting. 3. Nitrogen Blanketing of Internal Floating Roof Tanks SAC reviewed the results of the design meeting held by Jack Carlisle on this subject and recommended the nitrogen blanketing of internal floating roof tanks containing flammable liquids (having a flash point < 140F or is stored within 40F of its lowest reported closed cup flash point). The board accepted the recommendation and adopted this as a plant standard. 4. Silica Safety Association SAC had recommended earlier that a grant be given to support this association. Phil stated that if it was for dues, payment could be made locally; if it is a grant, approval by higher management in St. Louis is required. Trafton will be responsible for follow-up. 5. Warden Communications A PAFE has been approved for improving the warden communications system. SAC recommended that a test of the system be conducted as soon as possible after the new equipment had been installed. 6. Electrical Tracing Hazard Safety hazards arising from lack of control of certain kinds of electrical tracings are being defined by the SAC committee; however, the SHAC Notice of February 2, 1977 did not request a response from the recipients. A SHAC Action Request specifying the desired response should be issued to all manufacturing units. (This has been done by SHAC.) SAFETY HOUSEKEEPING AND ACTION COMMITTEE (SHAC) -- R. L. Stuart 1. Project "CONTROL CHART" SHAC has started a new control chart for keeping up with the projects, task forces, studies, plans and programs being conducted by the committee. This, control chart is expected to be helpful in showing the status of plantwide programs scheduled, underway, or completed by the committee. 2. "TAKE THE EXTRA STEP" Safety Program The program was launched on May 11 and preliminary results indicate the high degree of interest and enthusiasm. 002622 SC STG 2401944 LAM003829 PMSB Minutes - May 18, 1977 Page 3 3. Office Safety Committee Following members of the Office Safety Committee were appointed at the last meeting. Russ Furley, Chairman Ellen Harris Bob Wheeler Dorothy Boyd Bill Garrett Weldon Nonnenmacher Weldon was re-appointed to serve as the safety department repre sentative to the committee. 4. Annual Housekeeping Inspection The annual housekeeping inspection is the responsibility of the SHAC Committee. This possibly is the largest undertaking SHAC will have during the year. Because of its scope, the committee will be organized so that each member will have responsibility for a segment of the total effort. Also, SHAC will be asking many others to become involved by working on sub-committees. Bob Stuart will serve as overall chairman. 5. Review of Incidents The following incidents were reviewed by the Plant Manager's Safety - Board. Reports have already been issued. a. Methanol tankcar spill on 3/23/77 - Stuart b. Ammonia spill on 3/26/77 - Danner c. AN contamination of condensate system on 3/30/77 - Ryan d. Drott crane incident on 4/6/77 - McCullough e. Hexane fire - second level lines 1 & 2 on 4/22/77 - Daues f. Methanol-syn gas plant spill on 5/2/77 - Ryan g. Contractor incident (J. A. Jones rig contact with telephone cable on 5/4/77 - Martini 6. Fire Water Usage -- J. E. Clark The fire chief, John Eason, has reported that there have been entirely too many incidents of unauthorized use of fire water. The procedure bulletin, "Fire Protection and Equipment" (P-0124) clearly spells out the circumstances for using the fire protection water system.
