From Charles Ea ds \/ S u b je ct OSHA - REPEAT VIOLATIONS RE: YOUR MEMO ON SEPTEMBER 7 Cleveland, Tennessee I have compiled a list of our violations that were issued on March 14, 1972 and May 24, 1972. 1. Employees exposed to sound in excess of permissible levels without application of feasible administrative or engineering controls. (Example, of such excessive exposure were operators of the following compression mold grinders: 6-26, 6-27, Block width saw 2-6, Compression Mold saws 2-2 and the sandblasters 10-18.) . 2. Sound levels in excess of permissible levels without administration of continuing effective hearing conser vation program. 3. Employees exposed to asbestos dust in excess of five fibers per milliliters without the use of appropriate engineering methods of control. (Example of such exposure was the operators of Block briquette press 15-2.) 4. Employees exposed to asbestos dust in excess of permissible eight hour time weighted average without application of required engineering controls. (Example of such were operators of Disc briquetting presses exposed to eight hour time weighted average concentrations of 11.7 and 15.8 fibers per mi 11i1iter.) Reference to Item 1 : In our meeting here on September 18th with Oshex and Associates, Bob Andres, the following readings were taken and we are still in compliance with the present noise regulations. Sand Blasters Compression Mold Saws Block Saws 88 to 90 86 to 88 DB 86 to 88 DB The Compression Mold internal and external grinder readings were 92 to 94 with the old Bonitron hoods and 94 to 96 without the hoods. We made a new hood while Bob Andres was here on his visit, and lowered the noise level one DB. We should know within four weeks whether we are in compliance or if we need more engineering controls. (As of this writing we are not in compliance. You know the story without going into any more details.) Reference to Item 2 : A hearing conservation program was instituted on April 17, 1972 and is still in effect for todays regulations. Reference to Item 3 and 4 Engineering controls were installed in April, 1974 to reduce the employees exposure to asbestos dust below 2 fibers per milliliter and are still in effect today.
