There is , however, in d ire c t evidence, derived frost the Portland cement industry and from the gypsun industry, th a t exposure to dust containing lia s i s not significantly responsible for the formation of kidney stones, Ths Portland Cement Association, which includes most of the p lants in th s cement industry, keeps a careful record of accidents and illn e sse s of the approximately 50,000 employees o f the cement p la n ts. Although ths A ssociation's tabulation does not mention kidney stones sp e c ific a lly , i t i s shown in the la t e s t report ws have in our f i le s (1941) th a t th s frequency ra ts of diseases o f the genito-urinary system ( other than c ertain ones, lik e n e p h ritis, which are reported separately) was 2,5 fo r the cement industry as compared with 2,4 for a l l in d u stries reporting to the Public Health Servios, From th is comparison one would in fe r th a t dusts in esmsnt p lan ts, of which oalcium i s usually the predominating component, are not an important eause of kidney stones. As a p a rt o f our Investigation of the gypsum industry, in which calcine sulphate I s the p rincipal constituent o f th e atmos pheric d u st, we had occasion to make physical examinations of mere than 100 employees o f a c ertain p la n t. Vorwald, Director The Saranac Laboratory Saranac Lake, New York Dear Arts At one of our plants recently four men suffered kidney .stone attacks during an interval of a couple of days or so. "Everything of an in d u strial nature th a t might have contributed to these attacks was investigated, but the fin a l conclusion was th at the four occurred as a coincidence. One of the physicians consulted, however, has put fo rth the p o ssib ility th a t breathing lime dust might upset the calcium balance of the body and contribute to the formation of kidney stones. The plant uses lime as a raw m aterial and there i s a moderate exposure to lime dust--although we don't know as yet whether any of the four men involved had th is exposure or not. Has any work been published showing whether inhalation of lime dust i s lik e ly to re su lt in kidney stones? I don't believe th is was discussed in the cement-dust paper published by Dr. Gardner and others in 1939* but there may be something of more recent date th at we have missed. The suggestion was also made th at the drinking of coca-cola and other carbonated beverages might have had a bearing on the calcium balance. Trudeau Foundation In cooperation with; P h ilip C* P r a tt, t*Q* Tacce.s IL Durksn, M.S* Anthony B Dolakont Edward C. J . Urban, 11. This interim report, is e sse n tia lly a supplement to the previous re p o rt dated A pril 29, 1949* I t is submitted a t th is tin s in accor dance w ith a plan to issue rep o rts a t in terv ale of about six months. In the in te r e s t of b rev ity general statem ents as to the methods used, and purposes of each phase cf the experiments under ray ere omitted and the reader is referred to the preceding report fo r d is cussions of these points* I t may be stated th a t the conduct of the experiment has been maintained as i t was in e a r lie r stages, and th a t dust counts during the in te rv a l 3ince th9 l a s t re p o rt have averaged 179 m illio n p a rtic le s per cubic foot of air* In general, the observations cade over the past six months conform to those noted previously. However, only te n ta tiv e conclusions are warranted a t th is time. I t seems clea r th a t only a f te r a l l animals have been examined a t the end of the f u l l exposure can the re s u lts b9 evalu ated* The re s u lts to date are presented in tab les as in the previous report* C ertain comments concerning each ta b le follow : Table 1 (Compare with Table 3 .) In the in te rv a l since the l a s t re p o rt, four additional experimental animc.ls on being sa c rific e d have shown spreading of th e ir lesions while the con tro ls have consistently revealed healing or healed lesions. Others of the experimental group have fa ile d to show evidence cf spread. While the observations over this period suggest a slig h tly adverse influence of th e dust on the course cf the tuberculous in fec tio n , the evidence is by no means conclusive, and only subsequent developments and a survey of the re s u lts in the e n tire series of animals can pro** vide a basis fo r a positive statement as to the influence of Kaylo on a simultaneous tuberculous infection. Table 2. (Compare with Table. 3) Two of the three animals in phase I (in dust 2 months a f te r infection) of the re activ atio n experiment which were k ille d a fte r 12 months in the dust room showed gross evidence of spreading disease. This observation re mains to be oonfirmed by microscopic study. In the other two phases (in dust 4 months and 6 months ifter infection),no spreads have been noted. Here, too, the evidence is obviously incomplete and does not warrant evaluation a t th is time. Table 3. (Compare with Tables 1 and 2.) Ho spreading disease has been noted in the six animals k ille d since the la s t re p o rt. The course of the infection in the controls is thus seen to be in aocord with previous experience. Table 4 . (Compare with Table 5 .) The table reveals th a t the animals sacrificed or dying a fte r dust exposure of eleven or more months have co nsistently harbored spreading lesio n s. While some of these are yet to be confirmed m icroscopically, the consistency of the findings suggests rather strongly that under those conditions (infec tio n in itia te d a f te r 3 months of dust exposure) Kaylo has a d is tin c tly adverse influence on the course of the disease. Table 5 (Canpare witii Table 4 .) The oontrol animals again reveal th a t the infection is f ollovriag i t s usual course* One animal* k ille d a t 10 months a f te r in fe c tio n , showed s lig h t microscopic evidence of spreading disease* As noted in the previous re p o rt, i t is not uncommon fo r an occasional control animal to develop progressive disease following th is type of infection* In view of the above findings and fo r purposes of th is re p o rt, i t seems appropriate to present fo r comparison the chemical and petrographic analyses of Kaylo and of long f ib e r asbestos (Table 6). CCMLSHT At the present time the evidence sugg9s t 3 th a t Kaylo dust m y have a s lig h tly adverse e ffe c t upon tuberculous infection* This e ffe c t is by no means as pronounced as th a t of quartz, since sim ilar experiments with quartz re s u lt in progressive spreading of the lesions in v irtu a lly a l l animals a f te r 8 or more months of exposure. The lesions in animals exposed to quartz dust are usually s u ffic ie n tly extensive to cause death of some animals in the period from 12 to 15 months* The fu tu re course of the lesions in the present experiment cannot be predicted* They may heal during the next 9 months, but i t is f e l t somewhat more lik e ly th a t animals to be sac rificed w ill shew an increasing incidence of spreading disease* -5- EAYLO + ?.x 5ILTJLIMEOS inim al dumber 4 /; i V ( 2 6 3 ' 5 24 25 10 ( 7 / ; 8 so 14 22 1 s : 13 28 (11 12 17 20 ' 15 ] 6 Mode Of Death Duration.
