Mr. Bill Bridenbaugh spoke to the committee on the recent hydrotesting of No. 5 boiler in Power 2. During this test, carpenters and insulators were in the boiler when the coils were pressured with water for hydro testing. The increased pressure on the coils caused "creaking and popping" which startled the workers inside. Mr. Bridenbaugh said that this condition was caused by misunderstanding on the part of new maintenance supervision. Future hydrotests will follow a soon-to-beissued procedure which specifies no one inside during initial pressuring, and a five minute waiting period after test pressure is reached before personnel may enter to inspect for leaks. Personnel will also leave the boiler before depressuring the coils, because certain conditions during depressuring can also cause startling noises. Chairman Joe Lonsford made comments to the committee regarding the favor able impression of our Annual Inspectors' with the extent of involvement and contribution of our hourly personnel to the Texas City plant safety program. SHAC Chairman C. M. Cruse reviewed with the committee the comments of the Annual Inspectors at the feedback session following our inspection. New Business Some areas in the plant, and in particular Department 35P, are in need ^ of relamping. Two entries have been made in the Unit Safety Log with MWO's written each time. Relamping has not occurred. Concern was also expressed that lighting seemed inadequate in some areas when fully lamped. John Andrews will pursue this item in the unit, and seek assistance from John Fox in determining adequacy lighting in areas of concern of Depart ment 35P. sc 17112 LAM025364 John Andrews, et al -2- October 1, 1980 Elastic Safety Glasses Retainer Strap Discussion was held regarding the merits of the use of an elastic strap attached to the temple pieces of safety glasses to prevent slippage and dislodging of glasses. This item would be particularly advantageous during hot weather when perspiration is heavy. C. M. Cruse will investi gate this with LPSS. Combustible Alarm Testing Jessie Butler reported to the committee on the procedure for testing of the Department 51 combustible gas alarms and sprinkler systems operated by the alarm systems. This subject arose at a recent off-plant safety meeting and was pursued as a matter of interest. Similar testing programs are used for other systems in the plant. Plant Wide Roach Problem Complaints have been received from across the plant that control houses, smoke shacks, and kitchens need more effective pest control to better control roaches. Baseboard spraying is not totally effective. Cabinets also need attention. This item is something unit supervision needs to monitor when exterminators are used. Additional Eye Protection A. E. Mofley related to the committee the recent days away from work injury in Department 15/16 caused by wire striking an employee in the eye. There was discussion regarding adding side shields to safety glasses to minimize future potential for such an injury. C. M. Cruse reported that SHAC was investigating alternate tie-down materials, and suggested de ferring this subject until SHAC completes its study. Transtackers This is some holdover business from a previously completed item. One of the transtackers in Department 16 is inoperative. A. E. Mofley will pursue this item. Old Business Bicycles for HSC Members The request for special bicycles for HSC members was approved by the Plant Manager's Safety Board. Bicycles will remain assigned to the HSC. They will be ordered. Rubble Blocking Access to Substation 412-8 John Andrews reported that the access to the substation was cleared prior to the Annual Inspection. Since that time additional rubble has been deposited in the area leaving no drive access to the disconnect fuse pole. "C. M. Cruse will pursue with appropriate supervision. Mata of Capital Projects stating their procedures for handling ice. One problem is workers putting cold drink cans, etc., into the coolers. Some coolers recently purchased do not have provisions for locking the lids; they are being modified to permit locking. Barry Kane of CED stated that CED has purchased a crushed ice machine, and will be filling coolers at the machine, then transporting the coolers, rather than transporting ice to the coolers. This item is completed. Welding Restrictions at Research 2 Shop When Queeny is Docked This item was previously closed after having received verbal communication that this procedure will be modified. This month, that letter was received. This item is conplete. Falling Incandescent Light Vapor Barriers A letter was received from J. S. McCurley with recommendations and an action plan. Spare guards have been authorized for stores stocking. Guards in the field will be inspected during relamping inspections, and defective guards will be replaced as necessary.
