Conrad G. T. Ryan SHAC Paul Siems Bubba Bean The Fourth Quarter Safety and Housekeeping Inspection for Dept. 33 was con ducted on 1/24/84 by Paul Siems, Bubba Bean and Bob Hammann. Overall the unit was outstanding. Please respond to the following recommendations in writing to G. T. Ryan with car bon copy to Steve Moncla by 2/21/84. General Comments Should look at the storage of scaffolding on the top deck of the operating structure and also on top of CT8. Materials, especially on top of CT8, are of questionable condition. Personal Knowledge of Safety 1. Copy of the latest procedure bulletin P-0108 on Bicycle Safety was in the Safety Procedure manual in the control room. 2. New procedure on authority for bypass of critical alarms interlocks was well written and located in the control room. Need to follow through with plans for reviewing this procedure with all personnel. Personal Safety Equipment 1. Eyebath on second floor structure outside the control room blew a screen during a check by the chief operator. May need orifice replaced. Condition of Safety Equipment Safety chains were missing on chain operated valves next to D1 on second deck and on both Rl-1 valves. Safety Hazards 1. Ladder without an inspection tag on second floor of structure. 2. Scaffold without toe boards located southwest of control house in the rack. 3. Special wooden ladders behind the maintenance shop need to be inspected like other ladders. Discard broken or aged ones. 4. Toe plate missing on scaffold by southeast corner of CT8. 5. Need to upgrade insulation throughout the area. 3. Scaffolding lying on second floor structure. A couple of additional items were noted that had no effect on this inspection. 1. Step ladder from the paint shop is in the material storage building and has not been inspected. Needs to be returned to the paint shop. 2. Hearing protection signs are up and people seem to be aware of the require ment. Need to add an additional sign by the northwest side of CT8 pump area. /dgb R. T.
