UH/~oasZ* '* J '1 rJ June 8, 1979 TERMINATED EMPLOYEES' ASBESTOS MEDICAL SURVEILLANCE While employed with the Union Carbide Construction Department you worked in a craft, or the labor gang, which utilized insulation. It is an OSHA regulation that any terminated employee who was engaged in an occupation with possible exposure to airborne asbestos be offered medical surveillance. In compliance with OSHA, the medical surveillance will be available to all 1 aid-off employees who may have been exposed to asbestos. Should you wish to exercise this option a chest x-ray prior to an examination is required. An x-ray is available, at no cost to you, at Kanawha Valley Radiologists, Division Street,.South Charleston, (In Crigger Bldg.-across street from H. J. Thomas Hospital). The x-ray order is enclosed and their address and hours are stamped on the envelope. Upon completion of the chest x-ray, telephone the Institute Plant Medical Department (747-6212) for an appointment to complete the examination.
