MANAGEMENT SAFETY TOUR MEETING May 2, 1989 PLAINTIFF'S EXHIBIT ' RMC-2 4 AREA INSPECTED: Area I, II, III, IV & Powerhouse INSPECTION TEAM: J. Ferguson, K, Hester, D. Harriman, H. Hudson, W. Lew G. Coulter AREA I 1. #1 lime weigh belt needquite a bit of clean up in the silo. Quite a few hoses strung out at the reactor tank but they are being used as process lines (boiling out #1 settler). Scrap metal bucket missing at Station 30. At 42 lime silo a forklift bucket is being used as a garbage barrel. On 15B overhead belt there's a scaffold built at the drive motor (rotted away) - need to be taken down. 2. 90 oil storage area need to be straightened up. On the feed end of #1 rod mill is a scaffold. As far as major items, the area is looking much better. 3. Between ttte two south silos are some dust collector bag cages and other debris laying loose - need to be picked up; rusted table south west of 42 silo also a barrel that's been there a while; a cover off the top of a hatch that goes down into the tunnel was not flagged off (ground leva!). At the Bunker C oil area there's a bunch of wheel barrels stored (They are stored at proper place). At 22 there's a disconnect water fountain sitting beside the road. North of 15A upon the platform at the top conveyor quite a few timbers still laying on it outside the building - need to be brought down. North of Fac. 24 the acid station quite a few barrels in disarray and a 4" pipe that need to be picked up. 4. About an inch of alumina on the floor in 91 switch house. A portable light extension cord not in use hanging in the building the cord runs outside and plugs in underneath the unloader building - need to be taken out. Fire extinguisher need inspecting. At the South end of 15B - if they are going to continue to pump the sump out and let it run across and down the docks - we need to bury a pipe across the roadway. On the old sinter kiln there's 2 hoppers, close to the far end, insulation dropping off on top of the hoppers, the ground and on the walkways - need to be picked up. A condulet cover off some electric lines going into the ground south end of lime process. Standing On last Week's Items Everything has been corrected except some of the rocks and mud on the pad. Still a lot of material in the area by 45-4 sump by the trays as well as under 9 and 10 tray - need to be hosed in. Upstairs looks fairly good. Powerhouse 1. On the top level of 2, 3 & 4 deairator found numerous pieces of asbestos insulation. At the bottom of #3 deairator storage tank on the platform, numerous pieces of asbestos insulation. On the east end of #3 hi pressure feed pump, several pieces of asbestos insulation. Barrel of Mobil of SCH630 did not have an MSDS number 2. On the bottom of 3 TG are some rags, bolts. On the east side of 7 .boiler, on the storm sewer drain, the square 2x2 grating, there's three down that aisle way that are rusting and some of the grating is missing. 3. Insulation material laying between 8 and 9 on the ground. The coupling guards on 8 & 9 FD fans, only half coupling guards, very simple to put a hand under the bottom - something needs to be done. The room below the gas fired TG on the west side - oil on floor - air hoses without a pin - a gallon jug of Betz oil - a trash barrel in front of fire extinguisher and a trash barrel in front of the switch house. On the south side of the gas fired TG, there's an acetylene bottle tied to the wall with a piece of wire. In the back part of the air compressor room, on the northwest wall, there's two switch panels with materials stacked in front of them on a permanent type basis. Fire-extinguisher leading into the Secretary's office has no tag - the one inside the supervisor's office was checked in 1985. In the super visor's office there's a double 110 outlet that has a special plug put in with six outlets, five of which are covered and one of those five goes to another outlet device which has six available - circuit is handling 9 plugs. Numerous calls have been made to the supplier of the Mobil SCH630 for an MSDS number - to date have not received. 005152
