Godinho - 039 RG-244 Bagger Operator - For Union Carbide Corporation Metals Division - King City, California - Employee No. 423 - C.S.D. 4-21-81 - So cial Security No. 558-47-7831 Subject: CAL/OSHA Asbestos Regulations, Title 8, Section 5208, Subparagraph (K), Employee Notification (New Section adopted 11/1/77) Dear Mr. Godinho: In accordance with CAL/OSHA Asbestos Regulations, Title 8, Section 5208, Subpara graph (K), Employee Notification, notice is hereby given that your exposure of as bestos fibers was in excess of 2.0 limits in January, 1983. Two (2) between 87 to 102 minutes, personal air samples were taken. The average asbestos fiber count for the two (2) samples was 3.8 fibers longer than 5 micrometers, (TWA) per cubic cen timeter of air. During the time of exposure, you were using an approved respirator. MESA-NI0SH No. TC 21C-170 Corrective action shall be taken immediately. (B) Exhaust ventilation systems are in good working order. Exhaust ventilation systems are checked each shift, (on-going) (C) Asbestos spills shall be vacuum cleaned properly, (on-going) (D) Air-filtering respirators are in use and worn in accordance with provisions of Section 5144. (on-going) (E) Change room and special clothing will continue to be provided to each employ ee. Employees will be required to vacuum clean their coveralls as it is needed, (on-going) (F) Laundering of asbestos-contained clothing will continue in accordance with Section 5208. (on-going) NOTE: We did not perform routine monitoring of our baggers in January because of a NIOSH study of our bagging operations. Our Niagara Falls laboratory col lected parallel samples with NIOSH, and dust count results were not avail able for several months. This notification letter is late because we in advertently failed to transfer the data from an outside source to our local monitoring records. I /
