Place of death (state) must be known so that a copy of the death certificate can be obtained to ascertain causa of death. (2) A listing of 2employees who were reported to the IBS in two or more successive first quarters (Forms 941) but ere not Included in our study and have no record of employment et your plant. It Is especially import ant that we determine whether or not these individuels should be in the study cohort. Perhaps thase men were transferred along with their records to another facility? (3) A listing of 2. employees who were uncovered as e result of our RS record verification for which ve have not been able to trace for vital status as of 12-31-76. Please include these with the list of unknowns thst Ar.sis Garrison is presently working on. It is understood thst you and Angie will do further checking on (1), (2), end (3) above.
