Dear Dr. Budd:
In April of 1985, I originally corresponded with you regarding the development of an appropriate rehabilitation program for Mr. William Price, who is a patient of yours. Mr. Price was diagnosed in 1983 with angiosarcoma and in August of that same year, surgery was performed by you, which consisted of removal of part of the liver. It is my understanding that Mr. Price was last evaluated by you and blood work obtained in August of this year.
As you may recall, my job with Upjohn Health Programs/ Rehabilitation Services is that of a field-based case management coordinator. We attempt to work with the attending physician to develop and implement an appropriate individualized rehabilitation program.
I have scheduled an appointment to meet with you on 10/2/87 to discuss Mr. Price's current medical status. In April of 1985, he participated in a physical therapy rehabilitation program. At this point, I would like to discuss any recommendations you could offer for additional rehabilitation interventions.
Additionally, on October 2nd, a representative of DiversiTech General, a GenOorp company, who is Mr. Price's original employer, will be attending the appointment with me. They are in need of information regarding his current medical status and any additionally anticipated future medical intervention.
I thank you, in advance, for your time and attention regarding these matters. I look forward to meeting with you on September 21st.
