Tullier DATE January 10, 1984 SUBJECT Unreported VCM Release On December 23, 1983, a VCM release occurred in the VCM storage area (see attached Environmental Report). This incident was not reported or logged by shift supervision, and only one of the two operators involved logged the occurrence. Fortunately, a day supervisor in VCM-I read the operator's log book and reported the incident to the Environmental Group on 12/27, at which time they reported it to the State DNR four days late. This is not acceptable performance by operators or supervision. We are required by law to report such incidents on a timely basis. I cannot over emphasize the importance of good communications, both written and verbal, among operators, operators and super vision, and among supervisors. Please remind your people of the need for communicating and our legal obligations to report such occurrences.
