Box 5588 Manchester, NH 03108 Comp Unit WORKERS' COMPENSATION SEP 5 1991 RE: File No.: Sherwin-Williams 487 CBBM8 33921 Dear Kris: CTED On August 28, 1991, I spoke with Tony Colangelo at your insured. He said that the claimant's wages for the calendar year of 1989 (the fifty-two weeks prior to the January, 1990 injury) probably totalled $42,667.85 as outlined in the 1989 W-2 earnings summary. This includes the bonuses and commissions that Mr. received during calendar year 1989. On another note, Tony said that he would like to have copies of the medical records by Drs. Hurst and Hoffman forwarded to Sherwin-Williams so that they can quickly analyze them and determine whether or not their consulting occupational physician has any quarrel with the conclusions contained therein. You will recall that our own independent medical examiner agreed with Dr. Hoffman that this is a "classical" case of exposure to isocyanates and that Mr. 's exposure to isocyanates in the paint that he sold for Sherwin-Williams probably caused his asthmatic condition. Tony said he will let me know within a week if the consulting physician at Sherwin-Williams disagrees with the conclusions of Drs. Hoffman and Hurst. We will continue to keep you apprised of developments. Very truly yours, fas RNH/slhc cc: Anthony Colangelo Richard N. Hewes ^ N40238 C007-SWP-005802839
