Type and Purpose of Inspection Evaluation of construction stormwater permit compliance. IV. Facility Description The site lies in Beverly, MA, at the west end of Boulder Road, an unpaved dirt road leading west from Grapevine Road (which is across the town line in Wenham, MA). The site lies about 20 yards north of Route 128 (with a chain - link fence blocking access). Boulder Road ends at the southeast corner of the site. A cell - phone tower in a small fenced enclosure is located just north of the end of the road. The site slopes steeply then more gradually up to the north, with a lower spot in the northwest corner sloping down away to the west, a lower spot in the northeast corner sloping down to the east, and an old dirt road sloping down away from the site on the eastern side. About 50 yards east of the site, a small stream (flowing along route 128) crosses south under Boulder Road through a culvert and flows north. At the time of the inspection, the site had been cleared and earthmoving is underway on most of the site. V. Inspection Mr. Spejewski called Mr. Corsetti the afternoon of May 6 and informed him of EPA's inspection. Mr. Corsetti said there would be someone on site for the inspection. A. Opening Conference Mr. Spejewski met Mr. Buchsbaum at the intersection of Boulder Road and Grapevine Road, and they drove to the site. There they met Mr. Corsetti. Mr. Spejewski presented his credentials and gave Mr. Corsetti a copy of the EPA Small Business Information Sheet. Mr. Corsetti explained the site was owned by Oak Forest Farms, LLC, which was owned by his father. Mr. Corsetti owns Empire Construction. According to Mr. Corsetti, the project began as cutting trees for firewood, then " expanded. " Mr. Corsetti explained it was mostly to keep the company busy as there was little other work for them at the moment. He said the plan is to put in a greenhouse. (Later in the inspection, Mr. Corsetti 2 ED_019088A_00020085-00002 explained site access: saying he had an access easement along Boulder Road, even though the owner of the property the road crossed was unhappy with the easement.) Mr. Spejewski asked about an NPDES permit. Mr. Corsetti produced a copy of a printed flowchart for deciding whether a construction stormwater permit was necessary. The flowchart was marked with highlighter on the path for more than an acre disturbed but no discharge to a water of the U.S. Mr. Corsetti said he was familiar with " SWPPPs " but concluded one was not necessary for this site as there was no discharge. B. Facility Tour The site was cleared and grubbed, with earth stripped in most places. The south edge of the site, along the bottom of the hill, was mostly piles of logs, with a narrow undisturbed section then a chain link fence between the site and the ditch along the north side of Route 128. In one spot near the west end, signs of flowing water and sediment deposit led up to a small gap in the logs; there was no sign at the time of the inspection of sediment deposited beyond the edge of the site. The southern part of the site was a steep hill leading up to the north. This area was irregular with many small hollows and depressions, several of which were filled with water. The only area appearing to have concentrated flow was to the west, leading to the spot at the site boundary noted above. Along the lower (southern) part of the west edge of the site, a silt fence was in place, with a grassy area outside of the silt fence leading to a vernal pond. The site was mostly at or slightly below the terrain outside the fence. Farther north the site was bordered by undisturbed woods. The silt fence ended with a trench (apparently designed for silt fence installation) leading north and then ending. At one spot, a dip along the site edge led to a slope down to the west. Very little of the (currently cleared) site drained to the dip and there was no sign of sediment carrying off the site at that point. To the north of the site was woods, generally at or above the level of the site. To the northeast, another dip led to a slope down to the east. A small pond / large puddle was in place on the site just inside the border. No signs of flow from the pond to the site boundary were seen. Farther south along the eastern edge, an old dirt road led steeply down away from the site. No signs of significant flow to the road were seen. [following observations made while leaving the site:] Just east of the site a flowing stream came from the east along Route 128 and passed north through a culvert under Boulder Road, flowing into and apparently through a wetlands north of Boulder Road. At the intersection of Boulder Road and Grapevine, Boulder Road was unpaved, with no gravel or other site entrance, and dirt tracked out onto Grapevine Road. 3 ED_019088A_00020085-00003 C. Records Review No records were reviewed. D. Closing Conference Mr. Spejewski noted to Mr. Corsetti that he did not see any sediment discharge from the site at that time; however, Mr. Corsetti may wish to obtain permit coverage in case there was a future discharge. Mr. Spejewski and Mr. Buchsbaum left the site at about 11:15, pausing to observe the stream east of the site and the intersection of Boulder and Grapevine Roads.
