Scope: In paragraph 1, the phrase "2501 M Street, N.W., Washington, DC, 20037" is amended to read: "1300 Wilson Boulevard, Arlington, VA, 22209." The phrases "VCHC-4.0-EPI-UPDATE, ADDENDUM A, Data Transfer Delays and Replacement of Missing Study Materials, January 15,1996." and "VCHC-4.0-EPI-UPDATE, ADDENDUM B, Extension of Mortality Update Through December 31,1994, January 15, 1996." are added as additional tasks for Contractor to perform to the end of paragraph 3, Terms of Payment: The phrase "shall not exceed $346,225." is amended to read: "shall not exceed $437,395." The phrase "Six payment(s)" in item (b) of paragraph 9 is amended to read: "Eight payment(s)". In item (c) of paragraph 9, "$26,818" is amended to read: "$31,818". In item (d) of paragraph 9 "$315,113" is amended to read: "393,863". In item (e) of paragraph 9, "$31,112" is amended to read: "$43,532". Presentation of Study Results: In paragraph 15, the phrase "by April 1,1997." is amended to read: "by December 1,1997." In paragraph 16, the following lines should be added to the table: "02-1-97 07-1-97 Progress Report 6 Progress Report 7 $36,875 $36,875" In the table in paragraph 16, the two lines "01-1-97 04-1-97 Draft Report & Draft Manuscript $26,818 Final Report and Manuscript $31,112" are amended to read: "09-1-97 12-1-97 Draft Report & Draft Manuscript $31,818 Final Report and Manuscript $43,532".
