A Interoffice Communication To Distribution From A. H. Sather REVISED Date October 11, 1982 Subject VCM OVEREXPOSURE CONTROL MEETING 10/1/82 A meeting of this group (CRM, PEM, VEM, DLM, JVU, AHS) met on 10/1/82. The status of the following items was reviewed. I. VCM Overexposures A. The overexposures since the last meeting were reviewed along with a summary of overexposures by shift. The shift summary is attached for review. B. It was found that a number of overexposures had occurred during leak detection operations and there was no indication of respira tor usage. Corrective action should include review of the re quirements for respiratory protection equipment and counselling. C. Supervisors should plan to play a more active followup role to correct respiratory protection deficiencies as they occur in the field including the use of disciplinary action, D. VCM leak surveys will be routinely conducted following shutdowns in vinyl processing areas. The survey will be conducted by Opera tions with assistance from Safety and Engineering when requested. II, Honeywells Per DLM A. G.C 1) Old Module - All streams verified by sample* 2) New Module - Memory calibration required, pump parts that failed have been repaired. 3) Tank Farm - A work order to replace this tube bundle for sample collection has been issued* MSR is doing labor estimate. Sample tubing is being held by the supplier to provide rapid shipment. Estimate of time to place order and install are to be provided by DLM* 4) Lab G.C. - No action at present. Several points are to be relocated accoridng to DLM's last plan* DLM to initiate work order and send memo to State of Mississippi regarding intent of relocation and to confirm authorization per previous discussions with, the State, B. The strobe beacon is believed to have better visibility than existing Beacons. VAB.0001024773 l A Distribution 10/11/82 VCM Overexposure Control Meeting 10/1/82 Page 2 II. Honeywells Per DLM (Continued) C. Computer - The unit has remained on line for about six weeks since its last crash down. Minor modifications remain to be done. III. Filter Packs New line blinds are being installed. Piping revisions are complete and spool pieces can be removed for simplified installation of the lime blind units when they arrive. DLM & AHS suggested that contaminated filters be loaded directly into a dumpster and taken to the open area south of the new module (south of 4th street). The specific location will be recommended. IV. Hazardous task warning signs that warn personnel to stay out of a contami nated area will be specified and ordered by JVU. Operations must specify storage location for the signs to minimize mis-communication. % V. In-Process Leak Taps DLM and AHS have requested that special taps with valves and bleeder plugs be installed so that leaky valves can be readily identified. Leaks of this source are believed to be a possible cause of VCM overexposures. A. H. Sather Dist.: JF, CRM, PEM, VEM, DLM, JVU, CRS VAB.0001024774
