The process lead to the existing level and high level alarm in struments on the fresh vinyl chloride receivers have been electrically heat traced and insulated. February 2, 1979 Relief Valve Discharge 1. The standard operating procedure has been revised such that if after any charge of 5385 type resin the reactor pressure reaches 145 psig, the polymerization will be terminated with the emergency nitric oxide kill system. 2. Operating personnel are now required to wear Scott Air Paks before starting the addition of nitric oxide to the reactor. 3. The emergency nitric oxide kill systems for all reactors have been pressure checked to assure the integrety of the systems. 4. The emergency nitric oxide kill systems will be pressure checked every six (6) months. 5. The systems which transport the nitric oxide to the reactors will be checked each shift to assure the lines to the reactor are not plugged. 6. The standard operating procedure has been changed so that after every coarse batch, the reactor condenser will be inspected to determine if it is plugged. The standard operating procedure has been changed such that after every coarse batch, the initiator in the next batch is reduced by one-half gallon. 8. A new relief valve will be purchased and installed to replace the valve which did not reset after the pressure in the reactor decreased to less than the set pressure of the relief valve. 9. Operator training sessions covering the standard operating procedures have been held and will be conducted in the future on a formal basis once per year. February 10, 1979 Relief Valve Discharge 1. New rupture disc holders which can be assembled and pretorqued prior to installation will be installed beneath the relief valve. 2.
