Calgary Energy, LLC employee, Toby Jones was present for portions of the inspection. This was an EPA inspection in response to an initial spill inspection (NRC No. 1425812) conducted 3/11/2025. The inspection was conducted by the authority of the Clean Water Act (CWA) to determine compliance with Environmental Protection Agency (EPA) regulations. There was no other federal, state, or local agency representative present at the time of the inspection. FACILITY DESCRIPTION EPA conducted a follow-up inspection at the Calgary Energy, LLC, Tidal Osage 1 Lease, where a 3-inch steel flowline ruptured releasing a company estimated 350 barrels of saltwater and 2.5 barrel of crude oil onto the ground. The spilled fluid migrated south into an unnamed tributary, then west eventually impacting Little Hominy Creek. Section II - OBSERVATIONS I observed the confluence of the impacted, unnamed tributary with Little Hominy Creek. Cleanup was ongoing during the inspection. I observed significantly less oil on the tributary than during the initial spill inspection conducted 3/11/2025. See photo No. 1. At the low water bridge, I observed the floating, oil absorbent containment boom with crude oil being held back from further downstream migration. From this same location, along the bottom of Little Hominy Creek in the main channel, I observed a TDS field reading of 15,086 mg/L. This was the highest TDS observed during the inspection but did represent an improvement over the initial spill inspection observations. Se photos No. 2 & 3. I observed oil and oil sheen migrating downstream from the county road 5420 bridge crossing Little Hominy Creek at GPS (36.521017, -96.398967). This represented a downstream migration of spill impacts more than 1.5 miles (when following the creek channel) beyond the observed impacts from the initial spill inspection on 3/11/2025. I observed Calgary Energy, LLC employees installing containment booms on Little Hominy Creek approximately 2.5+ miles (when travelling in a straight line) downstream from the previously documented spill impacts. Booms were installed at GPS (36.495917, -96.394297). 2 Section III - AREAS OF CONCERN Calgary Energy, LLC / Tidal Osage 1 Lease CWA Inspection Date 03/11/2025 The primary area of concern was the unsecured spill impacts which had continued migrating down Little Hominy Creek since the initial spill inspection conducted on 3/11/2025. Additional areas of concern included the ruptured flowline, brine/oil impacted soil and vegetation along the overland flowpath, the impacted waters and banks of the unnamed tributary and the oil/brine impacts in the main channel and along the banks of Little Hominy Creek. Section IV - FOLLOW UP EPA inspector spoke with Blaine Haag via phone during the inspection to inform Calgary Energy, LLC that spill impacts were not yet secure and continued migrating downstream. Calgary responded by sending men and equipment to install the boom location (GPS 36.495917, -96.394297) referenced in the observations section above. EPA inspector received a follow-up call from Connor Haag on 3/18/2025 in reference to Calgary Energy, LLC's plan to repair the compromised flowline which was the release point of the spill. EPA inspector will conduct follow-up calls, field meetings and inspections to ensure the impacted area is returned to "pre spill" condition.
