Dear Mr. Martinez:
Enclosed is an Information Request (Request) issued to the City of Las Vegas, pursuant to Section 308(a) of the Clean Water Act (CWA), 33 U.S.C. 1318(a), which authorizes the Environmental Protection Agency Region 6 (EPA) to request information required to carry out the objectives of the CWA. The purpose of this Request is to obtain Discharge Monitoring Reports for your City of Las Vegas Water Treatment Plant as required by your National Pollutant Discharge Elimination System Permit, Number NM0030341, for the period from October, 2012 to the present. Your response to the Request must be postmarked no later than thirty days from your receipt of this letter. Compliance with this Request is mandatory.
Please be advised that failure to respond to the Request within the thirty-day period, or provide full, complete, true and correct responses, may result in the filing of an enforcement action by the U.S. Department of Justice, seeking the issuance of a court order directing you to properly respond to the Request. Please be further advised that provision of false, fictitious, or fraudulent statements or representations may subJect you to criminal penalties.

INFORMATION REOUEST
City of Las Vegas Water TreatmentPlant NPDES Permit Number: NM0030341
As owner/operator City of Las Vegas Water Treatment Plant, you are hereby required, pursuant to CWA, 42 U.S.C. 300j-4(a), to submit the following information pursuant to the Instructions set forth above:
1. With regard to the person providing answers to these questions, state your name, business address, business telephone number, and position with the City of Las Vegas Water Treatment Plant.
2. With regard to any person who participated in or contributed to your response to this Information Request, provide that person's name, business address, business telephone number, and position with City of Las Vegas Water Treatment Plant, including whether th~ person is an employee or a contractor.
3. Provide all City of Las Vegas Water Treatment Plant monthly Discharge Monitoring Reports (DMRs) as required by your NPDES Permit Number NM0030341 from October 2012 to present. In addition, future monthly DMRs should be similarly submitted. If the Facility has remained in a "no discharge" status, the October 2012 to present DMRs, referenced above may be submitted as follows:
a. Submit individually signed and dated No-Discharge DMRs for each monthly reporting period, or
b.. Submit a letter certifying that there have been no discharges from this Facility for the date range referenced in paragraph 3. The letter should include the certification statement signed by the authorized official on record.
4. Part I.B of the NPDES Permit (Monitoring and Reporting) states: monitoring results shall be recorded on Discharge Monitoring Report Form(s) EPA 3320-1 as specified in Part III.D.4 of the permit.
a. The permittee shall monitor and record the discharge(s) on DMR(s) monthly.
b. Quarterly reporting periods shall end on the last day of March, June, September, and December.
c. The permittee is required to submit regular DMRs as described above postmarked no later than the 28th day of the month following each reporting period.
